일차 진료에서의 범불안장애의 진단과 치료

초록

범불안장애는 일차진료 현장에서 자주 만날 수 있는 매우 흔한 질환이다. 그럼에도 불구하고 다양하고 모호한 신체 증상을 나타내며 여러 공존장애가 병행하여 나타나는 수가 많아 정확하게 진단하고 치료하기가 용이하지 않다. 더욱이 이 질환은 만성화하여 심각한 사회적, 개인적 고통을 유발하는 경우가 많으므로 조기에 진단하여 치료하는 것이 매우 중요하며, 특히 많은 범불안장애 환자들이 먼저 찾게 되는 일차진료의의 역할이 막중하다. 일차 진료의는 범불안장애에 대한 교육을 실시하여 불안을 유발, 악화시키는 생활 습관을 교정하도록 돕도록 하며, 단기적으로는 환자의 신체 증상과 불안 증상 경감을 목표로 하고, 궁극적으로는 환자가 발병 이전의 기능 수준으로 회복하여 재발이나 합병증이 없이 자신의 역할을 다하는 것을 목적으로 약물치료와 비약물치료 두 가지 모두를 포함하는 치료를 수행하여야 한다. 본 종설은 일차 진료 상황에서 범불안장애의 진단과 치료에 대하여 체계적으로 개괄하고자 작성되었다.
