서울시 세이프약국 사업의 현황 진단과 정책 과제 : 사업 참여 약국 대상 평가 연구를 중심으로

이 연구는 서울시민에게 포괄적 약력 관리, 금연지지 및 자살 예방 서비스를 포함한 지역사회 서비스의 문지기(gatekeeper) 기능을 약국을 통해 제공하고자 서울시가 2013년 도입한 ‘세이프 약국’ 사업을 평가하고 개선방향을 도출하고자 연구를 수행하였다. 이를 위해 온라인 설문조사와 초점집단면접을 기반으로 한 정량·정성자료를 모두 활용한 혼합연구방법론을 적용하였다. 온라인 설문조사는 최근 2년 이상 세이프약국 사업에 참여하고 있는 약국장 중 66명이 응답하였으며, 초점집단면접은 서울시 약사회의 추천을 받아 서울시 3개 자치구의 약국장을 대상으로 실시하였다. 분석결과, 응답자들은 10년 이상의 약국 운영 경력자(79%)들로, 현재 사업 운영에 대해 대다수가 긍정적인 인식을 가지고 있었다. 2019년 기준 평균 77.6명을 사업대상자로 운영 중이었으며 주로 포괄적 약력 관리와 가정보관 의약품 상담(88%)을 비중 있게 운영하고 있었다. 이들은 ‘새로운 약사서비스의 개발’과 ‘커뮤니티 케어 서비스 참여’, ‘약국의 공익적 역할’, ‘환자와의 신뢰강화’ 등 실제 환자와 지역사회에 기여한다는 측면에서 사업을 높이 평가하고 있었으며 환자 건강개선의 기여 및 사업 참여에 대한 자긍심 측면에서 높은 만족을 보였다. 이에 반해, 상담대상자 선정 및 동의서 구득 과정이 가장 어렵다고 응답하였고, 사업 운영 소프트웨어 프로그램에 정보 입력의 고충을 토로하였다. 세이프 약국 사업에서 지속적으로 추진해야 할 핵심 세부사업으로 포괄적 약력 관리(68.2%)를 꼽았고, 자살 예방 게이트 키퍼 사업을 재고해야 할 사업으로 꼽았다. 사업 활성화를 위한 최우선 과제로 사업에 대한 홍보(37.9%) 및 수가 개선(19.8%)를 꼽았다. 이에 본 사업으로 발전하기 위해, 사용자 편의적인 프로그램 개발, 적극적인 사업홍보 및 멤버십 도입, 사업의 목표 재고, 서비스의 선택과 집중, 직능갈등 완화를 위한 시 차원의 활동 등을 정책과제로 제안하였다.
Abstract
This study aims to evaluate ‘SAFE Pharmacy’ project, which was introduced to provide comprehensive drug utilization review, smoking cessation supports, and suicide prevention services as community gatekeepers in 2013, A mixed-method analysis based on an online survey (n=66) and focus group interview (n=3) was applied to evaluate the project and draw future directions. Respondents were those with more than ten years of experience in pharmacy management (79%), and most of them had a positive perception of the project. As of 2019, They have provided services to 77.6 people on average, mainly comprehensive medical history management and unused drug consultation. They showed high satisfaction in terms of pride in contributing to patients and communities, specifically ‘development of new pharmacist services,' ‘participation in community care services,' ‘pharmacy's role in the public interest,' and 'reinforcement of trust with patients.' On the other hand, they had the most difficulty selecting counseling subjects and obtaining consent forms and complained about entering information into the project software program. Comprehensive medical history management (68.2%) was selected as the core sub-project to be continuously promoted; however, gatekeeping for suicide prevention was selected for the most ineffective sub-project. Public advertisement (37.9%) and service reimbursement (19.8%) were selected as the top priority for future directions. We suggest that developing user-friendly programs, public advertisement of the project, introducing patients' membership, adopting a concentration strategy for selecting services, and alleviating professional conflict are needed for project scale-up.
