Abstract
The aim of this study was to investigate and assess the disease burden of neck pain using a nationwide health insurance data. Medical records spanning 20 years (2002–2021) were extracted for 105,504 individuals who visited healthcare facilities for neck pain (ICD-10 code: M54.2) from the South Korean National Health Insurance Database. The selected independent variables included age, sex, type of health insurance, health insurance premium percentile, and regional healthcare vulnerability score. The dependent variable, representing disease burden, was defined as the total number of healthcare service utilizations (hospital visits) where neck pain was recorded as the primary diagnosis during the study period. A one-way analysis of variance (ANOVA) was conducted to analyze differences in healthcare service utilization across variables. The average number of healthcare visits showed an increasing trend with age, with an average of 3.8 visits in individuals in their 20s, 4.2 in their 30s, 4.7 in their 40s, 4.9 in their 50s, and 5.7 in those aged 60 and above (p < 0.05). Female patients exhibited a higher frequency of healthcare utilization compared to males (p < 0.05). Patients covered by medical aid had the highest average number of visits (6.7), significantly more than other insurance types (p < 0.05). Additionally, individuals in the lowest income percentile (the first decile) recorded the highest average healthcare utilization, with 5.3 visits (p < 0.05). No specific trends in the regional healthcare vulnerability score were identified (p = 0.16). This study provides evidence for healthcare providers regarding the demographic characteristics associated with a high burden of neck pain, and highlights the need for future research on prevention and management strategies tailored to specific population groups to reduce disease burden
