Abstract

The purpose of this study was to identify the characteristics and risk factors of neck pain recurrence using the National Health Insurance Data Sharing Service that utilizes a nationwide cohort in South Korea. We extracted 2 years of medical records from the National Health Insurance database for 1,182,230 patients who visited healthcare services with neck pain (ICD 10 codes: M54.2) in 2020. Selected variables included age, gender, health insurance premium decile, regional health vulnerability index, duration of first episode, length of stay in healthcare, and the use of physical therapy and drug treatments at the first episode. Multivariate logistic regression analysis was conducted to analyze the recurrence rate and risk factors for neck pain recurrence. Among the subjects, 230,259 individuals (19.5%) experienced a recurrence of neck pain within two years, with a higher recurrence rate observed in elders and females. The regression analysis revealed that the risk of neck pain recurrence increased with older age (OR=1.58), being female (OR= 1.11), being a medical aid recipient (OR=1.45), residing in a region with a high health vulnerability index (OR=1.20), and longer duration of first episode (OR= 1.02). Conversely, higher health insurance premium decile (OR=0.95), longer stay in healthcare (OR=0.95), and receiving drug treatment (OR=0.89) were associated with a decrease in neck pain recurrence. This study contributes to improve understanding of neck pain recurrence and provide evidence of focused and long-term treatment plans for the prevention and management of neck pain recurrence for healthcare providers.
