의료시장의 개방이 일반적인 시장의 개방과 다른 점은 실질적인 개방의 전제가 일국의 의료시스템의 변화를 요구하고 있고, 따라서 의료정책의 변화와 연동된다는 점과 의료시장의 특성에 의하여 경쟁양상이 다르다는 점이다. 즉, 의료시장의 특성상 개방의 효과가 의료시스템의 변화와 직결되어 정책 환경의 변화가 불가피하다는 점과 의료시장의 특성에 의하여 경쟁양상이 다르게 나타난다는 것이다. 본 연구의 목적은 의료시장특성변수와 의료시장개방이 국가의료체계와 시장의 경쟁양상의 변화에 미치는 영향을 파악하기 위하여 의료시장개방에 대한 인식, 병원의 경쟁력, 의료시장의 특성과 병원의 대응, 정책적 대응 간의 관계를 우리나라 병원의 의사, 운영책임자, 행정책임자를 대상으로 구조방정식을 이용하여 실증적으로 분석하고자 한다. 본 연구의 결과는 다음과 같다. 첫째, 병원의 대응방안에 영향을 미치는 요인을 규명하기 위한 회귀분석에서 시장개방에 대한 인식도와 과잉시장규제인식이 정(+)의 유의적인 요인으로 나타났다. 둘째, 정책적 대응방안에 영향을 미치는 요인을 파악하기 위한 회귀분석에서 의료보험 개선과 경영성과 사이에는 경영성과가 정(+)의 영향이, 시설투자와 서비스 사이에는 부(-)의 영향이 있는 것으로 나타났다. 셋째, 병원의 대응방안과 정책적 대응방안 사이의 영향을 분석하기 위한 회귀분석에서 병원전문화와 의료보험개선에는 정(+)의 영향이, 병원의 대형화와는 부(-)의 영향이 있는 것으로 나타났다.
Abstract
When the market is opened, in regard with the medical policy, it is somewhat inevitable that the medical service should move from the public model of the United Kingdom to the sheer competitive system of U.S.A. Or, it should require such many institutional changes as the introduction of medical institutions of profit-making corporations, the introduction of private medical insurance and as the result the abolishment of the system which any medical institutions should compulsorily admit the national health insurance, the change of the insurance premiums, the abolishment of strong regulations including advertisement, and elimination of strong barriers to entry. The results of the investigations are as follows;The medical workers recognize the established medical policies as undue regulations and they consider market model's roles prior to those of the medical traits. They support that the speed of market open should be controlled for the small hospitals. In regard with the introduction of private health insurance, they support the mutual complementary system between them, which is meaningful for making new policies.
