의료시장 개방에 대한 인식과 병원의 경쟁력에 관한 연구

본 연구의 목적은 우리나라 의료시장의 개방에 대한 인식을 검토하고, 나아가 병원의 경쟁력을 실증적으로 분석하여 의료시장 개방 시에 어떻게 대응할 것인가에 대한 대안을 제시하고자 하는 것이다. 이를 위해 의료서비스의 공급자인 병원의 의사, 행정책임자, 운영책임자 등 전문가 집단을 대상으로 실증적으로 조사하였다. 일반적으로 시장의 개방은 국내시장에 대한 공급을 증가시켜 경쟁을 통한 생산성의 효율을 개선하는 것으로 알려져 있다. 그러나 의료시장은 그 특성상 개방의 효과가 다르게 나타날 것으로 전망되고 있다. 즉 의료서비스의 특성상 개방의 효과가 의료시스템의 변화와 직결되어 정책 환경의 변화가 불가피하다는 점과 의료시장의 특성에 의하여 경쟁양상이 다르게 나타난다는 것이다. 연구결과 시장개방의 긍정적 효과로 서비스수준의 향상과 경쟁을 통한 경영체질의 강화가 예상되었으며, 개방이 가져올 효과로 특히 경영환경의 급격한 변화가 예상되어 경영과 관련된 대응방안을 마련해야 할 것이다. 의료 시장개방에 대한 부정적 효과에 대해서 ‘중소병원의 경쟁위기’, ‘고급의료에 대한 선호도 심화’, ‘국민 의료비의 증가’에 대한 우려감이 높은 것으로 나타나 개방에 이은 중소병원의 경영위기가 심각한 것으로 보고 있고, 또 현재 사회 양극화가 심한 상황에서 개방으로 인해 의료 양극화 현상도 나타날 것으로 예상되는 바, 이에 대한 대책이 시급함을 시사하고 있다. 고급 의료에 대한 선호도가 증가하는 것은 결국 국민 의료비의 증가를 가져와 경제적 대립인 사회 양극화와 의료비의 과잉소비로 낭비적인 요소도 표출될 것으로 예상된다.
Abstract
The paper has investigated in a positive way what the effects of medical market's open will appear in the domestic medical market and the responding strategies among a group of the medical service suppliers including medical doctors, hospital executives and staffs. In general, it is known that market open of any field will bring higher productivity through increasing competitiveness which is possible by more supplies. However, it is expected that the medical market's open will show different effects due to the traits of it. That is, in order to bear any effect of the market open it is inevitable to change the medical service system and continuously change the policy environment, and eventually the competitiveness of the medical service will be different from that of other fields like economic areas. In this paper the author sets as independent variables: the degree of recognition about the market open, the traits of medical market, and competitiveness of hospitals. And then through the ways of resulting factors analysis it tries to reveal the cause and effect with the factors of the strategies both in the hospitals and political measures. The results of the investigations are as follows; To begin with, it is assured that producers of medical commodities prefers the market model to the public model. They suggested such measures are necessary to cope with new environments that the medical market will be changed into market oriented one and the public roles will be reduced.
