초기 대퇴골두 무혈성 괴사에서 수술적 치료시 고관절 관절경의 유용성

초록

목적: 대퇴골두 무혈성 괴사로 관절 구제술 및 인공관절 전치환술을 시행한 환자에서 고관절 관절경의 유용성을 알아보고자 하였다. 대상 및 방법: 1995년 5월에서 2004년 3월까지 초기 대퇴골두 무혈성 괴사로 관절경적 검사와 수술적 치료를 시행 받고 최소 1년(범위, 1.3~9.5년)이상 추시 가능한 16예를 대상으로 수술 전?후 임상소견, 방사선 소견 및 관절경 소견을 비교하였다. 결과: 자기공명영상에서 발견하지 못하고 관절경상 확인된 퇴행성병변은 대퇴골두 관절면 변화 11예, 비구 관절면 변화14예였다. 비구순의 퇴행성파열은 12예로 병변을 제거 후 추시상 서혜부 통증의 완화를 보였다. ARCO 병기 IIIa는 총 8예로 이중 골두 연골골절과 관절면 소실이 있었던 3예는 관절경 검사 후 인공관절 전치환술을 시행하였다. 관절구제술을 시행한 13예 중 수술 후 1년 이내 골두가 함몰되어 인공관절 전치환술을 시행한 경우는 5예였다. 결론: 고관절 관절경은 대퇴골두 무혈성 괴사에서 자기공명영상으로 확인할 수 없는 병변 및 동통의 원인으로 사료되는 관절내 병변의 진단과 치료를 동시에 시행할 수 있으며, 관절경상 골두 관절면의 손상정도에 따라 관절구제술이나 인공관절 전치환술 등의 치료 방법을 결정하는데 유용한 기준을 제공할 것으로 사료된다.

Abstract

Purpose: To evaluate the usefulness of hip arthrocopy in avascular necrosis of the femoral head (ONFH), which performed salvage operation or total hip arthroplasty (THA). Materials and Methods: From May 1995 to March 2004, 16 cases who were diagnosed ONFH and got operation under arthroscopy were the subjects. Follow up periods was at least one year (range, 1.3-9.5 years). Clinical,radiological and arthroscopic findings were compared before and after surgery. Results: Degenerative lesions which were identified by not MRI but arthroscopy were femoral cartilage changes in 11 cases and acetabular cartilage changes in 14. Labral tears was in 12 cases, after removal pain was relieved during follow up. ARCO stages IIIa were 8 cases. Among them, 3 cases, which showed head's delamination with articular cartilage fracture got THA after arthroscopy, Among 13 cases, who got salvage operation, 5 cases were reoperated with THA due to collapse within a year. Conclusion: Hip arthrocopy can be useful in the diagnosis and treatment of joint lesions, which can not be identified by MRI and which are causes of pain in ONFH. Hip arthroscopy can provide useful criteria in treatments such as salvage operation or THA depending on the femoral cartilage status.
