고령에서 발생한 불안정성 대퇴 전자간 골절의 수술적 치료

초록

목적: 고령에서 발생한 불안정성 대퇴 전자간 골절의 치료 결과를 평가하여 효과적인 수술방법을 모색하고자 한다. 대상 및 방법: 2002년 1월 부터 2003년 7월까지 대퇴 전자간 골절로 수술 받은 70세 이상의 환자로 1년 이상 추시가 가능하였고, 골절 유형이 Kyle-Gustilo 분류 4형 이상의 불안정성 골절인 30예(Kyle-Gustilo 분류 4형-A군 22예, 5형-B군 8예)를 대상으로 수술 후 고정 실패 양상 및 임상 결과를 후향적으로 평가 하였다. A군에서는 12예에서 압박 고나사로 고정술을 시행하였으며, 10예에서 양극성 반치환술을 시행하였다. B군에서는 전례에서 양극성 반치환술을 시행하였다. 결과: A군에서는 압박 고 나사로 고정한 8예에서 고정 실패를 보였다. 고정 실패를 보이지 않은 4예에서 골유합을 보였으며, 양극성 반치환술에서는 7예에서 안정적인 고정을 유지하였다. B군에서는 1예에서 후방 탈구, 1예에서 대전자부 불유합이 있었으며, 6예에서 안정적인 고정을 유지하였다. 수술 전후 보행 능력 회복 정도는 압박 고 나사 고정에 비해 양극성 반치환술 후 그 결과가 양호 하였다. 결론: 고령에서 발생한 Kyle 분류 4형에서는 압박 고 나사 고정술이 양극성 반치환술에 비하여 결과가 불량하였으며, 5형의 양극성 반치환술의 결과는 비교적 양호하였다. 따라서 고령에서 발생한 Kyle-Gustilo 4, 5형과 같은 불안정성 대퇴 전자간 골절의 수술적 치료로 양극성 반치환술이 유용할 것으로 사료된다.

Abstract

Purpose: To find out the effective operative treatment by evaluating the operative reuslts of unstable femoral intertrochanteric fracture in the elderly. Materials and Methods: Among the patients of femur intertrochanteric fracture from 2002 January to 2003 July,we have chosen 30 cases who were over 70 years old, able to follow up over 1 year, and whose fracture type was over the Kyle-Gustilo type 4, and have done surgical treatment, then retrospectively evaluated the fixation failure and clinical result. In group A, compression hip screw (CHS) was used in 12 cases, and bipolar hemiarthroplasty in 10 cases. In group B, bipolar hemiarthroplasty was done in all cases. Results: In group A, 8 cases using CHS showed fixation failure. 4 cases showed union. In bipolar hemiarthroplasty, 1 case showed posterior dislocation, 2 trochanteric nonuion, and 7 stable fixation. In group B, 1 case showed posterior dislocation, 1 greater trochanteric nonunion, and 6 maintained stable fixation. The recovery rate of walking ability before and after surgery, bipolar hemiarthroplasty was better than CHS fixation. Conclusion: Fixation using CHS in Kyle-Gustilo type 4 in the elderly, the consequences were poorer than the bipolar hemiarthroplasty, and in type 5, bipolar hemiarthroplasty's consequence was somewhat satisfactory. So we conclude, for the surgical treatment of unstable femur intertrochanteric fracture like Kyle-Gustilo type 4, 5 in the elderly, bipolar hemiarthroplasty would be a better choice.
