이극성 반치환 성형술이 시행된 환자에서 고관절 탈구는 수술 후 생길 수 있는 합병증으로, 비관혈적 도수 정복 시에 드물게 폴리에틸렌 내피와 인공 골두 사이의 내측 관절면의 분리가 발생할 수 있고, 이러한 경우 반드시 관혈적 정복을 필요로 한다. 내측 인공 골두가 폴리에틸렌에서 분리되는 힘을 측정해보고자 자체 제작한 모형으로 압박실험을 시행한 결과도수 정복과정에서 충분히 가해질 수 있는 힘으로 분리되는 것으로 판단할 때, 이극성 반치환 성형술 후 탈구된 환자의 정복 시 인공 골두와 폴리에틸렌 내피의 분리를 방지하기 위해 충분한 마취와 근 이완을 시킨 후 조심스러운 도수 정복이이루어져야 하며, 도수 정복 후에는 방사선 사진의 정확한 비교를 통해 인공 관절의 연결 상태를 정확히 확인해야 한다. 저자들은 이러한 드문 후유증 2예를 경험하였기에 이를 보고하고자 한다.
Abstract
In this two cases presented here, dissociation occurred between polyethylene liner and the inner ball head after closed reduction of dislocated bipolar hemiarthroplasty of the hip. Closed reduction without anesthesia dissociated the intraprosthetic joint. To measure the force that causes the inner head to separate from the polyethylene, we constructed a model of bipolar hemiarthroplasty and carried out mechanical test. This force can be likely caused to that which occurs during reduction of the dislocated joint of a patient who has undergone bipolar hemiarthroplasty. If hip dislocation occurs after bipolar hemiarthroplasty, we suggest that closed reduction occurs after bipolar hemiarthroplasty, we suggest that closed reduction be carried out while the muscles are relaxed under anesthesia.
