선천부신과다형성 환아에서 부신겉질호르몬 치료 전후에 발생한 이차성 중추성 성조숙증 2례
Two Cases of Secondary Central Precocious Puberty in Boys with Congenital Adrenal Hyperplasia - before and after Treatment with Corticosteroid

가성 성조숙증 환자에서 시상하부-뇌하수체-생식샘 축의 성숙을 동반한 진성 성조숙증이 발생할 수 있는데 이를 이차성 중추성 성조숙증이라 한다. 저자들은 이차성 중추성 성조숙증이 발생한 선천부신과다형성 남아 2명을 경험하였다. 환아들은 선천부신과다형성으로 진단받았으며, 한 명은 부신겉질호르몬 치료 후 진성 성조숙증이 발생하였고, 또 다른 한 명은 치료 전에 이미 진성 성조숙증이 동반된 상태였다. 기존의 히드로코르티손과 9α-플루드로코르티손과 함께 추가적인 생식샘자극호르몬분비호르몬 작용제로 치료를 하였으며 관련된 문헌고찰과 함께 보고하는 바이다.
Abstract
Central precocious puberty (CPP), with early maturation of the hypothalamic-pituitary-gonadal axis, may develop in patients with pseudo-precocious puberty, which is secondary central precocious puberty. We have experienced two such boys who had diagnosed as congenital adrenal hyperplasia. These children had been treated with physiologic dose of hydrocortisone and 9α-fludrocortisone. One patient have diagnosed as CPP after treatment and the other, before treatment. They have been treated with gonadotropin releasing hormone agonist (GnRHa) when they developed CPP.
