연구배경 : 담배 의존도 척도 (Cigarette Dependence Scale: CDS)는 Fagerstrom 니코틴 의존도 척도 (Fagerstrom Test for Nicotine Dependence: FTND)의 단점을 개선하여 12항목 및 5가지 항목으로 축약된 버전이 있으나, 우리나라에서 아직 검증되지 않은 상태이다. 방 법 : 검진 센터에 방문한 흡연자를 대상으로 FTND와 CDS-5 설문, 소변 코티닌 검사를 시행하고 흡연력을 조사하였다. FTND와 CDS-5의 상관관계를 확인하고, 내적 일치도를 Cronbach's alpha 계수로 평가하였다. 설문항목의 소변 코티닌과의 상관관계를 조사하였으며, CDS-5의 각 항목별 분포를 조사하였다. 분석은 연령별, 흡연량별로 따로 구분하여 분석하였다. 결 과 : 총 245명이 설문을 완성하였으며 이중 남성이 237명, 여성이 8명으로 남성의 자료만 분석하였다. 평균 연령은 48세였고, 평균 흡연량은 1.1±0.5갑이며, FTND와 CDS-5의 평균과 표준편차는 각각 4.2±2.4와 18.0±3.4였다. CDS-5의 내적일치도는 0.720로 FTND의 0.633보다 높았으며, 소변 코티닌과의 상관계수도 CDS-5가 0.461로 FTND의 0.422보다 약간 높았다. 특히 CDS-5는 연령군에 관계없이 0.7 안팎의 높은 내적 일치도를 보이는 반면, FTND는 특히 50세 이상 군에서 0.53의 값을 보였다. 결 론 : CDS-5는 높은 내적 신뢰도와 소변 코티닌과의 상관관계를 보였고 연령에 따른 영향도 적어 적극적인 활용이 필요하다.
Abstract
Background : The Cigarette Dependence Scale (CDS-5) was developed by Etter et al. to measure the degree of a smoker's dependence. This may improve the weakness of the Fagerstrom Test for Nicotine Dependence (FTND). In Korea, there has not been any study to verify the validity of CDS-5. Methods : We administered the FTND and the CDS-5 at one health checkup hospital. Urine cotinine was measured to test its correlation with each test. In addition, the internal consistency of each test was confirmed by Cronbach's alpha coefficient. We analysed the distribution of the items in CDS-5 and conducted factorial analysis of the questionnaire. We also did a stratified analysis according to age groups and smoking amount. Results : Of the 245 subjects, 237 were male and 8 were female. The mean±standard deviation scores for FTND and CDS-5 were 4.2±2.4 and 18.0±3.4, respectively. The internal consistency (Cronbach's alpha) of CDS-5 was 0.720, which was higher than that of FTND (0.633). The correlation with urine cotinine was also higher for CDS-5 (r=0.461) than for FTND (r=0.422). While the Cronbach's alpha of FTND was merely 0.53 in the 50 and older age group, that of CDS-5 was about 0.7, irrespective of age. Conclusions : The internal consistency and correlation with urine cotinine were better for CDS-5 than for FTND. Therefore, there is a greater need to use and to study the CDS-5.
