구강건강이 여성 노인의 일상생활수행 능력, 우울 및 삶의 질에 미치는 영향

초록

연구배경: 노인의 구강건강은 노인의 신체적, 정서적 건강에 영향을 주기 때문에 매우 중요함에도 불구하고 구강건강이 노인의 건강이나 삶의 질에 미치는 영향에 대한 연구가 미흡하다. 따라서 본 연구의 목적은 노인의 구강건강영향지수와 일상생활수행 능력, 우울 및 삶의 질에 대하여파악하여 노인의 삶의 질 향상을 위한 전략 개발에 기초자료를 제공하기 위함이다. 방법: 자료수집 기간은 2013년 12월 1일부터 12월 31일까지이며, 본 연구의 대상은 65세 이상의 여성 노인 201명이며, 인구사회학적 특성, 구강건강 영향지수(oral healthimpact profile [OHIP-14]), 일상생활수행 능력(activity ofdaily living [ADL]), 우울 및 삶의 질(quality of life[QOL])을 설문조사하였다. 인구사회학적 특성, OHIP-14,ADL, QOL의 상관성을 분석하기 위하여 SPSS 20.0 프로그램을 이용하여 평균, 표준편차, 카이제곱과 t-test, ANOVAtest, Pearson’s correlation coefficient로 분석하였고, 가설적 모형에 대한 적합도는 AMOS 20.0 프로그램을 사용하여 검증하였다. 결과: 구강건강 영향지수는 0.71점이었고, 우울은 6.34점, 삶의 질은 2.12점이었다. 구강건강 영향지수는 학력, 경제수준에 따라 차이를 보였다. 학력이 낮은 노인(F=7.49,P=0.001)과 경제수준이 낮은 경우(F=4.08, P=0.018), 질병이 있는 경우(F=3.08, P=0.048) 구강건강 영향지수가 높았다. 또한 우울은 학력(F=7.49, P=0.001)과 경제수준(F=4.08,P=0.018)에 따라, 삶의 질은 학력(F=7.01, P=0.001), 경제수준(F=4.87, P=0.009), 질병(F=11.04, P<0.001)에 따라 차이를 보였다. 일상생활수행 능력은 구강건강 영향지수에영향을 주었고(r=0.414, P<0.001), 구강건강 영향지수는우울에 영향을 주었으며, 삶의 질에는 우울을 매개로 한간접적인 영향을 미치는 것으로 나타났다. 결론: 노인의 건강 증진을 위하여 구강건강에 관심을 가져야 하며, 노인 구강건강 관리에 대한 대책을 수립하는것은 노인의 우울을 감소시키고 삶의 질을 향상시키는 전략이 될 수 있다.

Abstract

Background: Although oral health in the elderly significantly affects their physical health and quality of life, therehave only been limited studies on the relationship between oral health and quality of life. Thus, the purpose ofthis study was to examine the relationships between oral health, ADL, depression and quality of life in the elderlyin Korea, and to provide data to assist the development of health promotion programs in this population. Methods: Study subjects included 201 community dwelling elders recruited during December 1 to 31, 2013. Data collected included socio-demographics, oral health (OHIP-14), activity of daily living (ADL), depressionstatus (GDS-SF) and quality of life (QOL). For data analysis, ANOVA test, t‐test, Pearson correlation coefficientand multiple regression analysis were performed using the SPSS 20.0 and AMOS 20.0. Results: Average score of OHIP was 0.71, depression was 6.34 and QOL was 2.12. There were significant differencesin OHIP according to education (F=7.49, P=0.001), economic status (F=4.08, P=0.018) and diseasestaus (F=3.08, P=0.048). Also there were statistically significant differences in the depression status accordingto education (F=7.49, P=0.001) and economic status (F=4.08, P=0.018), and in QOL according to education(F=7.01, P=0.001), economic status (F=4.87, P=0.009) and disease (F=11.04, P<0.001). ADL influenced OHIPand depression, OHIP influenced depression and depression influenced QOL. Conclusions: Oral health in the elderly should be considered in developing health programs for quality of life.
