초록

연구배경: 건강정보 활용능력은 질병 유병률 및 만성질환자 자가 관리에 있어 중요한 개념으로 의료비용 및 부정적 건강결과에 영향을 미침에도 불구하고 국외에 비해 국내에서는 이와 관련한 연구가 부족한 실정이다. 이에 본연구는 지역사회 거주 일반 성인을 대상으로 건강정보 활용능력의 수준과 건강정보 활용능력에 영향을 미치는 요인을 파악하기 위해 시도된 횡단적 조사연구이다. 방법: 본 연구의 대상자는 서울, 경기, 충청지역에 거주하는 만 18세 이상의 성인 420명으로, 자료수집 기간은2011년 11월부터 12월까지로, 일대일 면접에 의한 자가보고형 설문형태로 자료를 수집하였다. 건강정보 활용능력은Chew 등이 개발한 Short Test of Functional Health Literacy in Adults 도구를 사용하였다. 자료 분석은 Predictive Analysis SoftWare (PASW) 18.0 프로그램을 이용하였다. 결과: 대상자의 건강정보 활용능력 수준은 최대 75점 만점에 평균 50.64점(±19.18)으로 나타났다. 회귀분석 결과, 대상자의 건강정보 활용능력에 영향을 미치는 요인으로는 최종학력이 고졸 이상일수록(ß=0.17, P=0.001), 음주를 하지않는 사람일수록(ß=-0.12, P=0.010), 지각된 건강상태가 좋을수록(ß=0.11 P=0.029), 건강정보 활용 능력 수준이 높음을알 수 있었다. 이들 세 변인의 설명력은 약 7%로 나타났다. 결론: 본 연구결과를 바탕으로 건강관리자들은 건강관련 교육 시 대상자들의 건강정보 활용능력 수준을 파악하고, 의료기관의 서식 및 교육자료 등의 개발에 있어서 대상자의 최종학력, 건강행위, 건강상태 등을 고려하여 건강정보의 이해와 활용을 강화할 수 있는 교육 및 의사소통전략을 수립할 필요가 있겠다. 향후 건강정보 활용능력에영향을 미치는 요인들의 명확한 기전을 밝히는 연구가 요구되며, 또한 광범위한 인구집단을 대상으로 건강정보 활용능력을 측정할 수 있는 한국형 도구의 개발이 필요하다고 할 수 있다.

Abstract

Background: Health illiteracy is a problem often unrecognized by health care providers. It influences medical costs and the health status of adults. The purposes of this study were to determine the level of health literacy in community-dwelling adults and to identify the factors influencing it. Methods: A cross-sectional survey was conducted in Seoul, Gyeonggi and Chungcheong province. A total of 420 adults aged 18 or older were interviewed by trained nursing students between November 1 to December 30, 2011. Health literacy was measured using the Short Test of Functional Health Literacy in Adults. Data were analyzed using the PASW 18.0 program. Results: The mean score of health literacy was 50.64±19.18. In the multiple linear regression analysis, health literacy was significantly associated with education (ß=0.17, P=0.001), alcohol use (ß=-0.12, P=0.010), and perceived health status (ß=0.11, P=0.029). These factors accounted for about 7% of health literacy. Conclusions: Health literacy is a very important public health issue. Our findings showed that educational level,alcohol use and perceived health status should be considered when assessing this issue in patients. Furthermore, the development of a standardized Korean assessment tool for health literacy and specified interventions for enhancing health literacy are needed to improve health outcomes.
