Abstract
This study systematically reviewed intervention studies on aquatic rehabilitation exercise for older adults with physical disabilities, with the aim of analyzing program components and effectiveness and providing directions for evidence-based program development. A total of 1,505 studies published between January 2005 and December 2024 were initially identified, and 11 experimental studies were finally included according to the PICO-SD criteria (Participants, Intervention, Comparison, Outcomes, Study Design). Most domestic studies employed quasi-experimental designs, whereas international studies primarily adopted randomized controlled trials (RCTs). The interventions commonly integrated aerobic, strength, and balance training, and consistent improvements were observed in key outcomes such as balance, lower limb strength, and cognitive function. These findings demonstrate that aquatic rehabilitation exercise is an effective intervention that enhances exercise accessibility and sustainability for older adults with physical disabilities. Based on these results, this review suggests the need for future research employing rigorous RCT designs, incorporating psychosocial variables, and conducting long-term follow-up studies. Furthermore, it provides foundational evidence for the development of community-based programs and policy support to promote sustainable aquatic rehabilitation for this population.
