연구목적: 폐경여성에서 녹내장 발생과 혈중 에스트라디올 (estradiol, E2)농도와의 관련성을 알아보고자 하였다. 연구재료 및 방법: 산부인과 외래를 방문한 30명의 폐경여성을 대상으로 혈중 E2 농도, 폐경 관련 임상 지표, 안압, 빛간섭 단층촬영 등의 녹내장 관련 임상 지표를 측정하였다. 안과적 선별 검사 결과에 따라 녹내장 의심군으로 분류되면 정밀검사를 시행하여 녹내장 여부를 확진하였다. 모든 환자들을 녹내장 의심군과 녹내장 비의심군으로 분류한 후 두 군 사이의 혈중 E2 농도 및 지질 농도, 골밀도 등의 폐경 관련 임상 지표들을 비교 분석하였다. 결과: 전체 30명 중 녹내장 의심군으로 분류된 환자가 8명 (26.7%)였으며, 이중 1명이 치료가 필요한 녹내장, 2명이 초기 녹내장으로 진단되었다. 녹내장 의심군과 비교하여 녹내장 비의심군에서 혈중 E2 농도가 더 높은 것으로 측정되었지만, 통계적인 유의성은 없었다 (19.40 ± 4.79 vs. 13.95 ± 4.55 pg/mL, P = 0.525). 혈중 E2 농도가 높은 군 (≥ 20 pg/mL)과 낮은 군 (< 20 pg/mL) 사이의 녹내장 의심자의 빈도 또한 유의한 차이가 없었고 (25.0 vs. 27.3%, P = 0.645), 녹내장 의심군과 비의심군간의 다중 로지스틱 회귀분석을 시행한 결과에서도 녹내장 의심군과의 유의한 연관성이 있는 지표는 역시 없었다. 결론: 폐경여성에서 빛간섭 단층 촬영과 같은 정밀 선별검사를 시행한 결과 일반적으로 알려진 비슷한 연령대의 녹내장 유병율보다 높은 빈도에서 녹내장 환자를 발견할 수 있었다. 혈중 E2 농도와 안압 및 녹내장 발생 위험간의 통계적 유의성은 확인할 수 없었다.
Abstract
Objectives: To investigate the occurrence of glaucoma and association with the serum estradiol (E2) level in postmenopausal women. Methods: We evaluated the serum E2 level, female reproductive factors and glaucoma related risk factors including intraocular pressure and optical coherence tomography (OCT) findings in 30 postmenopausal women who visited Obstetrics and Gynecology outpatient clinic. Patients who showed abnormal findings on the glaucoma screening test were classified to the glaucoma suspect group (group-G), and underwent a glaucoma confirmatory test. Serum E2 level, female reproductive and other menopausal health-related factors such as lipid profiles and bone mineral densities were analyzed in the group-G and non glaucomatous group (group-N). Results: Eight out of thirty participants (26.7%) were classified to the group-G. One of them was diagnosed as having glaucoma that required treatment, and the other two were found to have early glaucomatous changes. Compared to the group-G, the group-N had a higher level of serum E2 (19.40 ± 4.79 vs. 13.95 ± 4.55 pg/mL) The difference, however, was not statistically significant (P = 0.525). The proportion of glaucoma suspect patients in the groups with a higher serum E2 level (≥ 20 pg/mL) and a lower serum E2 level (< 20 pg/mL) was similar (25.0 and 27.3%, P = 0.645). Multiple logistic regression analysis showed that no female reproductive factors were associated with the risk of glaucoma. Conclusion: Comprehensive glaucoma screening using an OCT in postmenopausal women could detect more glaucoma patients than prevalence in the similar age group. Statistical significance was not found in the association between serum E2 level and the risk of glaucoma.
