목적: 폐경기 여성에서 병합 호르몬 대치요법이 안구건조증에 미치는 효과에 대해 알아보고자 하였다. 대상과 방법: 건강한 폐경기 여성 36명 72안을 대상으로 3개월간의 전향적인 추적관찰을 통해 쉬르머검사와 눈물막 파괴시간과 각막형광염색 및 자각증상검사를 호르몬 대치요법 시행 전과 시행 후 1개월, 3개월에 시행하여 각 검사치를 비교하였다. 모든 대상환자들은 에스트로겐과 프로게스테론으로 지속적 병합 호르몬 대치요법을 시행 받았다. 결과: 자각증상점수는 치료 전 2.10±0.79에서 치료 1개월 후 1.32±0.84, 3개월 후 0.88±0.67로 감소하였으며(p<0.001), 눈물막 파괴시간은 치료 전 6.04±2.54 초에서 치료 1개월 후 7.99±2.43, 3개월 후 8.93±2.20으로 의미 있게 증가하였다(p<0.001). 형광염색검사에서는 치료 전 0.94±0.71에서 치료 1개월 후 0.42±0.60, 3개월 후 0.22±0.42로 유의하게 감소하였다(p<0.001). 결론: 폐경기 여성에 있어 에스트로겐과 프로게스테론 병합 호르몬 대치요법은 안구건조증을 유의하게 호전시켜 줌을 알 수 있었다.
Abstract
Purpose: To evaluate the effects of conjugated hormone replacement therapy (HRT) on dry eye syndrome in postmenopausal women. Methods: Thirty-six healthy postmenopausal women (72 eyes) were included in this three- month prospective study. Schirmer test, tear film break-up time (BUT), fluorescein staining score and symptom score were performed on all patients at the time of enrollment, one month and three months after initiation of HRT. All patients received HRT consisting of conjugated estrogen and progesterone taken orally in a continuous combined regimen. Results: Symptom score significantly decreased from 2.10±0.79 before HRT to 1.32±0.84 after one month and to 0.88±0.67after three months (p＜0.001). In addition, tear film BUT measurements significantly increased from 6.04±2.54 seconds to 7.99±2.43 seconds after one month and to 8.93±2.20 after three months (p＜0.001). The fluorescein staining score was 0.94±0.71 before HRT and decreased to 0.42±0.60 after one month and to 0.22±0.42 after three months (p＜0.001). Conclusions: Hormone replacement therapy consisting of conjugated estrogen and progesterone significantly improved the symptoms and signs of dry eye syndrome in postmenopausal women.
