애보트 4세대 HIV 항원/항체 검사 시약의 평가

초록

배경 : 사람면역결핍바이러스(HIV) 감염과 HIV 항체 검출될 때까지의 혈청학적 진단전의 잠복창기를 줄이기 위하여 p24항원과 항체를 동시에 검출하는 HIV 항원/항체(Ag/Ab) 검사가 개발되었다. 최근에 새로 개발된 4세대 Architect HIV Ag/Ab combo를 다른 4세대 시약인 AxSYM HIV Ag/Ab combo와 3세대 시약인 AxSYM HIV 1/2/O와 함께 진단적 성능을 비교하였다. 방법 : 세 가지 혈청전환 패널과 건강검진자 305예와 HBsAg양성 혹은 항 HCV 항체 양성자 100예에 대하여 세 가지 시약으로 검사하였다. 정밀도 평가를 위하여 각 시약의 검사내 변이계수와 총 변이계수를 분석하였다. 결과 : 세 가지 혈청전환 패널들에서 Architect HIV Ag/Abcombo는 AxSYM HIV 1/2 gO보다 8.7± 2.1일, AxSYM HIVAg/Ab combo보다 2.0 ± 2.0일 진단 시점을 단축시켰다. 양성대조에 대한 총 변이계수는 Architect HIV Ag/Ab combo, AxSYMHIV Ag/Ab combo, AxSYM HIV 1/2 gO 각 3.6%, 9.3%,4.6%로 Architect HIV Ag/Ab combo가 장 우수하였다. 특이도 평가에서 세 가지 시약에 따른 차이가 없었다.
