자동면역측정기 ADVIA Centaur를 이용한 알레르겐 특이 IgE 검사의 평가

초록

배경 : 알레르겐 특이 IgE 검사는 알레르기의 진단과 치료에서 중요한 역할을 한다. 저자들은 ADVIA Centaur 장비에서 화학발광면역법을 이용하여 측정하는 알레르겐 특이 IgE 검사의분석능을 평가해 보았다.방법 : 한국에서 흔한 호흡기 알레르겐인 d1, d2, e1, e5, t3,t7의 6항목에 대해 정밀도, 희석회수율(평행성), PharmaciaUniCAP 특이 IgE 검사법 및 피부단자시험과의 결과 비교, 검체소요량, 분석속도 등을 NCCLS 지침(I/LA20-A, EP5-A2)에따라 평가하였다. 정밀도 평가를 위해서 상품화된 양성, 음성 정도관리물질을 사용하였고 희석회수율, 상관성 평가를 위해 총10명 환자로부터 얻은 검체를 이용하였다.결과 : 6항목의 검사내 변이계수는 3.45-6.14%였고 장비내 변이계수(총변이계수)는 모두 10% 이하였다. 희석계수간 변이계수는 2.84-11.95%로 측정가능범위 내에서 우수한 직선성, 평행성을나타냈다. UniCAP 특이 IgE 검사와의 일치율은 76.3-96.1%(d1 88.2%, d2 96.1%, e1 91.0%, e5 77.0%, t3 90.5% and t776.3%)이였고 피부단자시험과의 일치율은 모두 80% 이상이었다. 검체소모량은 상대적으로 적었고(검사 당 25 L), 처리속도가 빠르고(시간 당 120건) 검사소요시간이 짧았다(첫 결과 획득소요시간: 47분).
