초록
Implant-supported overentures for fully edentulous patients offer a solution to the stability and retention challenges faced by conventinoal complete dentures. This study aims to compare and analyze the clinical outcomes of mandibular implant-supported overdentures using solitary (nonsplinted, NS) and bar (splinted, SP) attachments. Over an average follow-up period of 11.43 years, a total of 82 patients (39 NS; 43 SP; average age 68.28 years) and 180 implants (86 NS; 94 SP) were included in the research. All patients experienced complications at least once, with prosthodontic complications being more frequent in the SP group, although no significant differences were observed. The most common attachment-related complication was loss of retention, with no significant differences between the groups. Peri-implant diseases showed significantly higher risk in the SP group (peri-implant mucositis HR 3.12; 95% CI [1.51-6.43]; peri-implantitis HR 4.19; 95% CI [1.20-14.61]), and all instances of osseointegration failure occurred in the SP group. The cumulative implant survival rate was 100% for the NS group and 96.8% for the SP group, with no significant difference. In the SP group, a significant increase in the risk of peri-implant disease was observed with a decrease in horizontal distance between implants.
