의약품 처방 규제 관련 정책의 문제점 및 향후 과제: 의약품 사용평가 제도를 중심으로

의약품사용평가(Drug Utilization Review, DUR)란 정해진 기준에 따라 약물사용양상을 해석하고 구조화하는 체계적이며 지속적인 노력을 의미한다. 구체적으로는 의약품 사용에 있어 서 적정 수준의 질을 확보하기 위하여 의약품 사용 자료를 수집ㆍ분석ㆍ평가하고 이에 따라 의사를 교육ㆍ시정하며 평가결과를 규명해가는 순환적인 임상과정이다. 현재 국내에서 시행되고 있는 의약품사용평가 시스템은 의약품 처방이 이루어짐과 동시에 그 내용을 컴퓨터에 탑재된 가이드라인과 비교하여 처방의 변경이 필요하거나 주의가 필요한 사항에 대하여 알려주는 경보시스템이다. 현행 의약품 사용평가 제도는 국민건강보험법과 동법 시행령에 근거하여 시행되고 있는데, 해당 제도의 관련 고시는 환자의 건강권 및 의사의 진료권을 침해할 우려가 있으며 국내의 현실에 부합하지 못하는 측면이 있다. 이에 본고에서는 의약품 사용평가제도의 개념 및 국내ㆍ외 제도현황을 살펴보고, 이를 근거로 현행 우리나라 제도의 문제점과 쟁점들을 살펴보았다. 결과에 따르면 가장 시급한 문제는 국내 현실에 부합하는 한국형 의약품사용평가(DUR) 체계로 개선되어야 한다는 것이다. 이를 위해 한국인의 약물반응에 대한 데이터를 축적하고, 이를 근거로 합리적이며 과학적인 의약품사용평가를 수행해야 한다. 특히 약물반응 데이터 구축에 있어서는 관련 전문 학회 및 임상전문가의 의견을 충분히 수렴하여 기준을 설정하고 지속적으로 보완해야 할 것이다.
Abstract
Drug Utilization Review, or DUR, is a circular clinical process that collects, analyzes and evaluates drug utilization materials, educates and corrects doctors as a result, and identifies evaluation results, in order to secure an appropriate level of quality in using drugs. The Drug Utilization Review(DUR) system now being operated in Korea is an alarm system by which the content of a prescription is compared with the guideline saved in a computer immediately after the prescription is written and matters need to be changed or pay attention, if any, are informed. It is apprehended that the current DUR system will infringe upon patients’ right of health and doctors’ right of medical treatment, as well as having an aspect not to accord with domestic condition. Hence, this study took a look into the concept of DUR system and the present condition of it in and out of the country and explored problems and issues of the current system being operated in the country. Findings indicate that it is the most urgent problem to improve the system into the Korean DUR system corresponding with domestic condition. For this, accumulation of data on Koreans’ drug response and reasonable performance of DUR should be made. Standards shall be established and continued to be complemented with collection of relevant experts’ opinions in full.
