본 논문은 개별 환자를 분석 단위로 하여 의원급 외래를 이용한 환자의 건당 진료비에 영향을 미치는 요인을 분석하는 것을 그 목적으로 한다. 환자의 성, 연령, 중등도와 같은 환자 요인 뿐 아니라 공급자의 성, 연령, 장비 보유 여부, 개원 지역, 경쟁 정도, 진료하는 환자의 전반적인 특성과 같은 공급자 요인도 독립변수로 포함하였다. 본 연구의 분석 자료는 2006년 상반기에 의원 외래를 이용한 모든 건강보험 및 의료급여 환자를 대상으로 한 본태성 고혈압, 무릎관절증, 배통, 인슐린의존당뇨 네 가지 외래 다빈도 질환이다. 분석 결과 의료급여 환자의 외래 건당진료비가 건강보험환자보다 더 높았으며, 환자의 연령과 중증도가 높을수록 건당진료비가 더 높았다. 공급자의 연령이 낮고 개원 기간이 짧을수록, 고가 장비를 보유한 의원일수로, 경쟁 정도가 높을수록, 진료하는 환자의 전반적인 중증도가 높을수록 건당진료비가 높은 것으로 나타났다.
Abstract
The purpose of this study was to examine the factors affecting medical expenditure associated with outpatient care provided in physician clinics. Besides patient characteristics, this study examined the effect of provider practice patterns on health expenditure. The study used the outpatient claim data for all Medicaid and health insurance patients of hypertension, gonarthrosis, dorsalgia and diabetes, who received medical services in physician clinics in the first half of 2006. Empirical results showed that outpatient expenditure per care is greater for Medicaid patients, and the age and severity of patients are positively associated with greater medical care expenditure. Provider characteristics, such as the age and length of practice of the physician, ownership of high-tech equipment, competition in the market area, and the severity and age structure of the patient pool, are associated with outpatient expenditure, too.
