2000년 7월 이후 정착된 우리나라의 단일보험자체제(single payer system)는 높아진 협상력을 활용하여 의료보장 급여 범위 확대 및 급여비 억제라는 상충되는 목표를 비교적 성공적으로 이루어 냈다는 평가를 받고 있다. 한편 다보험자체제(multi payer system)의 경우 보험자 - 의료공급자 간 시장 형성을 통해 기술 혁신이 보다 활성화되는 데 반해 우리나라와 같은 단일 보험자체제의 경우 기술 혁신이 제한적으로 이루어질 수 있는 문제점이 있다. 본 연구는 의료서비스 생산과 관련한 의료공급자들의 효율성 수준이 상이한 상황에서 Revelation Principle(Myerson, 1979)을 활용한 차별적 수가체계(differentiated fee schedule)를 도입할 경우 기존의 단일수가체계(uniform fee schedule) 보다 더 높은 사회후생이 가능함을 보인다. 특히 의료공급자의 수익수준에 변화를 주지 않으면서도 보험자의 순편익을 증가시키는, 파레토개선이 가능한 “실현 가능한 적정수가체계(feasible optimal fee schedule)”를 제안한다. 이를 통해 현행 국민건강보험공단의 단일 보험자 체제 하에서도 사회후생을 증가시킬 수 있는 의료서비스수가정책에 대한 정책적 시사점을 모색한다.
Abstract
Since the National Health Insurance Corporation(NHIC), the integrated single payer system has begun in July of 2000 in Korea, it has been considered that it has successfully achieved the dual goals of benefit expansion and cost containment, using the increased bargaining power. However, the NHIC may face the problem of insufficient innovation as most other countries with a single payer system generally has. In this paper I introduce the mechanism of differentiated fee schedule devised using Myerson(1979)'s Revelation Principle and show it outperforms the system of existing uniform fee schedule in terms of insurer's welfare. Especially the "feasible optimal fee schedule" developed in this paper shows the case in which Pareto Improvement can be made where the insurer's welfare can be increased with healthcare provider's profit unchanged.
