중․고령자의 미충족 의료 등의 발생요인과 비공식적 케어의 활용

본 연구에서는 중․고령자의 ‘의료 등에 대한 니드(need)’ 발생 요인을 알아보고, 이러한 니드(need)를 가지고 있으면서도 서비스를 충족 받지 못하는 사람들의 특성을 알아보았다. 이와 더불어 중․고령자의 케어 서비스를 공식적인 케어와 비공식적인 케어로 나누고, Anderson 모형을 바탕으로 각각의 케어 이용 행태에 미치는 영향 요인을 살펴보았다. 분석데이터는 2008년 고령화연구패널이었는데, 우리나라 중․고령자들은 ‘의료 등에 대한 니드’가 발생할 경우 우선적으로 공식적인 케어를 이용하고 있었으며, 비공식적인 케어도 보완적으로 이용하고 있는 것이 확인되었다. 하지만 경제적 취약층이나 고령층에서는 여전히 케어에 대한 미충족(unmet) 현상이 발생하고 있었다. 이러한 결과는 두 가지 시사점을 제시한다. 첫째, 중․고령자들이 공식적인 케어를 효율적으로 이용할 수 있도록 고령사회에 적합한 방향으로 현행 보건의료제도를 개선할 필요가 있다. 둘째, 독거노인을 비롯한 저소득 고령층의 케어 수요를 해소하기 위해서 사회복지시스템을 확충하는 한편, 비공식적인 케어를 제공할 수 있는 지역사회의 인력 자원을 풍부하게 개발할 필요가 있다.
Abstract
This study aims at examining the elderly's unmet needs for care and identifying factors influencing their use of care services in Korea. To find the influencing factors, two separate estimates are generated for formal care and informal care, based on Anderson model. For the empirical analysis, data are from the KLOSA(Kore Longitudinal Study of Aging) conducted in 2008. The analysis shows that formal care is preferred rather than informal care when care is needed. And informal care is an effective complement for formal care when formal and informal care are used collectively. The result indicates that the probability of experiencing unmet need is significantly influenced by socio-economic status such as gender, age, and the level of household earnings. These findings suggest two implications. First, it is required to reform the existing health care and welfare system to make it suitable for the Aged Society. Secondly, it is necessary to develop community human resources providing informal care effectively for the poor elderly, especially living-alone older people, who are trying to overcome the unhealthy conditions.
