의료비 가계본인부담지출 추계에 있어서의 균등화지수 활용방안

본 연구에서는 현재 우리나라의 가계본인부담지출의 주요 자료원으로 활용되고 있는 가계조사 자료를 활용하여 균등화 지수를 산출, 가계본인부담지출을 추계하는 방안을 제안하였다. 의료비의 경우 발생빈도가 낮고 분포의 표준편차가 크다. 그러므로 기존 통계자료의 집계치(가계조사의 공식발표자료)만을 활용할 경우 가계조사의 높은 가구 대체율로 인한 비표본추출오차 문제가 발생할 수 있다. 즉, 높은 가구 대체율로 인해 조사 완료 시점의 가구원 수 분포가 전수 분포와 달라지고, 조사대상가구의 조사개월 수가 각기 달라지기 때문이다. 따라서 본 연구에서는 기존의 가계조사 자료를 활용하되 조사대상가구의 높은 대체율로 인한 문제를 보정한다는 관점에서, 가구원 수를 고려한 의료비 지출의 균등화 지수를 산출하여 추계에 활용할 것을 제안하였다.
Abstract
The Household Income and Expenditure Survey data, which are analyzed in this study, have been the main sources for the estimation of household's out-of-pocket expenses for health in the National Health Accounts. Although household's out-of-pocket expenses for health is related to the family size as well as the scale of household consumption, the household's traits are not considered when the expenses are estimated. Given that health care expenditure is non-normally distributed and happens irregulary, the high attrition rates among the respondents of Household Income and Expenditure Survey can influence the estimation results by non-sampling error. In this study, we examine which calculation method of household's out-of-pocket payments for health is more appropriate for the adjustment of the differences between household size and its composition. We also derived equivalence scales for household's out-of-pocket payments for health, which are estimated from the health care expenditure data of Household Income and Expenditure Survey.
