일본 의료계획 방향 및 시사점

우리나라와 일본은 병상 공급 증가와 지역간 불균형 문제를 해결하기 위한 정책수단으로써 공통적으로 ‘계획’제도를 채택하고 있다. 일본의 의료계획 제도는 병상수 통제라는 ‘양적계획’으로 시작하여, 지역의료제공체계 구축이라는 ‘서비스 제공계획’의 형태로 발전하고 있는 반면, 우리나라의 병상수급계획 제도는 계획수립과 이행이 지지부진한 실정이다. 일본이 의료계획 제도를 도입함으로써 의도한 정책효과를 거두었는가에 대한 평가는 분명하지 않지만, 일본의 병상수는 하향 추세를 보이고 있다. 그 과정에서 일본 정부는 제도의 정비 외에, 각 지방정부에 필요한 기술적, 재정적 지원을 병행하고 있으며, 이해당사자들의 동의를 얻어냈다. 정책의 부재로 인한 병상수의 과잉과 의료전달체계의 붕괴 문제를 경험하고 있는 한국에 시사점이 크다.
Abstract
South Korea and Japan, both have experienced an increase and maldistribution in the hospital bed supply, and have a ‘Planning’ as a policy tool to solve the problems has been adopted. South Korea’s ‘Hospital bed supply and demand Plan’ is in a stalemate, While Japan's ‘Medical care plan’ is improving to the type of 'Care Planning' from the type of 'Capacity Planning'. It is not clear whether the 'Medical care plan' is effective to achieve the intended objective in Japan. However, the number of beds are decreasing in Japan. The Japan government has supplemented a policy, with technical and financial assistance for each regional government, and has obtained the consensus of the stakeholders. This conveys a strong implication to the South Korea's situation that has delivery system failure and over-supplied hospital bed.
