코로나19 유행 전후 의료이용 수평적 형평성의 변화

본 연구는 2020년부터 발생하여 전 세계적으로 유행한 코로나19의 국내 의료이용 상황을 탐색하고, 코로나19 발생 전과 비교하여 코로나 발생 이후 의료이용의 수평적 형평성 변화를 확인하는데 그 목적이 있다. 이를 위해 본 연구에서는 한국의료패널 2기(2019~2020) 연간데이터를 활용하였으며, 20세 이상 성인을 분석대상으로 포함하였다. 연구대상자는 2019년 11,537명, 2020년 10,509명이 선정되었으며, 종속변수인 의료이용은 전체 의료서비스, 입원 의료서비스, 응급 의료서비스, 외래 의료서비스의 이용 여부, 입내원일수, 의료비 지출로 설정하였다. 분석 방법으로는 의료이용의 수평적 형평성을 분석하기 위하여 집중지수(concentration index, CI)와 의료이용의 수평적 형평성 지수(Horizontal inequity index, HIwv index)를 산출하였다. 수평적 형평성 지수 산출 결과, 2019년과 2020년에 모든 의료서비스에서 저소득층에게 유리한(pro-poor) 불형평이 발견되었으며 2019년에 비해 2020년에 수평적 형평성 지수가 증가하여 0에 수렴하는 방향으로 변화하는 것을 확인할 수 있었다. 이는 코로나19 유행 상황에서 우리나라의 의료이용 형평성이 악화되지는 않았으나 2019년 저소득층에게 유리하였던 의료이용 형평성 지수가 2020년에 0으로 수렴해 코로나19 유행이 저소득층의 의료이용에 불리한 방향으로 적용하였음을 시사한다. 본 연구는 코로나19 유행 전후 수평적 형평성 지수의 변화를 살펴본 국내 첫 번째 연구라는 점에서 의의가 있다.

Abstract

The purpose of this study is to explore healthcare utilization during Covid-19, which has occurred since 2020, and to compare the horizontal equity in healthcare utilization before and after the outbreak of Covid-19. This study used the Korean Health Panel Survey(2019-2020) including individuals aged 20 and above. To measure horizontal equity in healthcare utilization, we employed the concentration index(CI) and horizontal inequity index(HIwv). The results showed that the HIwv index score was negative, which implies pro-poor inequity in 2019 and 2020, but it increased in 2020 compared to 2019 and changed toward convergence to zero. This suggested that in Korea, although the equity in healthcare utilization has not deteriorated during Covid-19, it has had negative impacts on low-income groups. The significance of this study is to be the first research to analyze the change in horizontal equity before and after the Covid-19 outbreak in Korea.
