소득계층에 따른 보건의료의 형평성:『한국노동패널』자료를 중심으로

본 논문에서는 『한국노동패널』 자료를 근간으로 하여 의료비지출과 건강수준의 측면에서 우리나라 보건의료의 소득계층별 형평성을 실증적으로 분석하였다. 소득과 의료비지출의 수직적 형평성을 측정한 Kakwani지수는 모든 연도에서 저소득계층에 불평등한 -0.30에서 -0.37 범위의 역진적인 값으로 계측되었으며, 이는 미국(1999년)의 -0.38과 비슷한 수준으로 상당히 높은 불평등도에 해당하며, 의료비지출 불평등도는 시간이 지남에 따라 증가와 감소를 반복하였다. 소득계층별 건강수준의 형평성을 계측한 CI+는 -0.028에서 -0.037정도의 값으로 계측되어 우리나라의 소득계층별 건강수준의 불평등도는 일본(1992년)의 -0.018과 비슷하며 불평등 정도는 그다지 크지 않은 편이었다. 건강수준의 불평등도는 시간이 지남에 따라 불평등의 정도가 줄어들다가 2005년부터 다시 그 불평등도가 커지는 양상을 보였다.
Abstract
Using annual survey data compiled from Korea Labor & Income Panel Study, we analyzed income-related inequalities in health care finance and self-assessed health. Over the 1999∼2007 period, Kakwani indices, measuring inequality in health care finance, ranged from -0.30 to -0.37 while displaying no clear temporal pattern. These values are comparable to -0.38, the corresponding value for the USA for the year 1999, implying high inequity/regressivity in health care finance. On the other hand, standardized concentration indices, measuring inequality in self-assessed health, ranged from -0.028 to -0.037 while displaying initially a downward trend in inequality up to 2004 and then subsequently an upward trend. These values are comparable to -0.018, the corresponding value for Japan for the year 1992, implying very low inequality in health.
