알콜문제로 인해 갈등을 겪고 있는 부부의 가족치료 사례연구

초록

본 연구는 알콜중독 문제를 가진 남편과 그 가족을 돕기 위한 가족치료 모델로서 MRI의 상호작용적 가족치료 모델과 Bowen의 가족체계 이론의 효과성을 보고자 하였다. 또한 알콜중독의 원인을 가족체계라는 맥락에서 이해하여 가족구성원들이 유지해왔던 시도된 해결책을 인식한 후 이를 탈피할 수 있는 새로운 대처 방식을 시도하고 유지함으로써 가족의 문제를 해결한 두 모델의 원리와 개념들을 분석하였다. 그리고 Miles와 Huberman이 개발한 시간에 따른 디스플레이 중 하나인 “사건 흐름 네트워크”를 활용하여 분석에서 발견된 개념들을 도식화하였다. 분석결과로서, 부부갈등의 주원인은 술, 미분화, 역기능적 대처방식이라는 3가지 범주가 도출되었고 부부갈등과 관련된 부부의 원가족 특성은 크게 남편의 원가족 특성과 부인의 원가족 특성으로 나눌 수 있었다. 남편의 경우 음주, 폭언, 폭력에 대한 다세대 전수가 있었으며 부인의 경우 부모로부터의 신체적, 언어적 학대 뿐 아니라 성폭력의 경험으로 인한 낮은 자존감과 피해의식이라는 심리적 특성이 남편과의 상호작용을 왜곡시키는 준거틀로서 작용하고 있었다. 가족치료의 결과로는 대화방식의 변화, 음주량의 감소, 인식의 변화, 역기능적 대처방식에 대한 비효과성 인식이라는 4가지 범주가 도출되었다.

Abstract

The purposes of the study were to find the causes of the conjugal conflict relating to husband with alcoholic problem, to examine the characteristics in family of origin regarding conjugal conflict, and to reveal the effectiveness of MRI model and Murray Bowen's family systems theory with regard to the result of the family therapy. The total family counseling was 14 sessions, which included individual, couple, and family counseling. The study used constant comparative analysis by using open coding method to find the causes of conjugal conflict, the characteristics in family of origin relating to conjugal conflict and the result of family therapy. The study used Miles and Huberman's time-ordered network display to show the result of the study. The results of the study showed alcohol, undifferentiation of self, dysfunctional communication pattern as causes affecting conjugal conflict. The characteristics in family of origin regarding to conjugal conflict were divided into husband's characteristics and wive's. For the husband, alcoholism, violent language, violence were transmitted from family of origin, while the wife had low self-esteem because of parents' physical and verbal abuse as well as sexual abuse experience. The psychological characteristics of these low self-esteem and victim's consciousness distorted the interaction with the husband. Finally, the results of the study revealed the change of communication skill, the decrease in dose of drinking, the change of cognition, and the insight of the ineffectiveness in dysfunctional coping mechanism as the results of family therapy.
