음주문제를 가진 성인자녀에 대한 가족치료 사례연구

초록

본 연구는 대학교 때부터 음주문제로 어려움을 겪고 있는 성인자녀와 가족에 대한 총 11회기 동안 이루어진 개별상담, 부모상담, 가족상담을 통해 가족치료의 개입방법과 효과성을 살펴보고자 하였다. 상담 축어록과 상담시 기록한 메모를 중심으로 개념과 하위범주, 중간범주, 상위범주로 나누었으며, Miles와 Huberman(1994)이 제안한 matrix와 network를 활용하여 정리하였다. 그 결과 성인자녀의 음주문제에 영향을 미친 요인으로는 가족 내 역기능적인 의사소통, 부정적 양육태도, 부모의 원가족 경험 및 역기능적 의사소통으로 나타났으며, 지금까지 문제를 해결하기 위해 시도되었던 방식이 문제를 지속시키고 있었다. 이에 대한 치료자의 개입방법으로는 문제 확인, 치료적 동맹, 재정의, 반복되는 패턴의 조명, 비유법, 새로운 의사소통 방식의 지도, 변화 확인 등을 사용하였다. 결론적으로 MRI의 상호작용적 가족치료모델과 Bowen의 가족체계이론을 통한 치료자의 개입방법은 내담자 가족으로 하여금 기능적 의사소통을 할 수 있도록 하였고 이를 통해 성인자녀는 음주를 조절할 수 있게 되었다.

Abstract

Objectives: This study aimed to explore factors influencing on the alcohol problem of a grown-up child, the therapeutic techniques for family therapy, and the change of family members after family therapy. Methods: The data was consisted of recording transcripts and note-taking of 11 session family therapy. The study used a constant comparative analysis, and Miles & Hubermann's matrix and network display as a analysis method. Results: The results of this study revealed dysfunctional communication pattern, parents' negative rearing attitude, parents' experiences form the family of origin, and dysfunctional communication pattern in the parents' family of origin as factors influencing on the alcohol problem of a grown-up child. The study found the formation of therapeutic alliance, confirmation of problems, relabeling, reflection of the repeated dysfunctional communication pattern, guidance of communication skills, sharing and comparing with similar cases, and therapist's self-disclosure, confirmation of change as therapeutic techniques of family therapist. Conclusions: The result of study showed that the family therapy enhanced family members' functional communication skills and decreased the use of grown-up child's drinking.
