본 연구는 ADHD 아동의 정신병리에 따른 신경인지기 능의 차이를 알아보기 위해 2006년 3월 1일부터 2007년 8월 30일까지 영남대학교 의과대학교 부속병원 정신 과 외래를 방문한 ADHD아동을 KPI-C의 하위척도 T 점수에 따라 발달지연군, 내재화군, 외현화군, 중복군, ADHD 단독군으로 분류하여, 이들의 지능지수와 CNT 를 비교하였다. 발달지연군의 지능수준은 다른 군에 비해 유의한 차 이가 있었으며, 청각적 연속수행검사의 누락오류 역시 다 른 군에 비해 유의하게 높게 나타났다. 선추적 검사 A, B에서 소요시간, 오류수에서 다른 군에 비해 높게 나타 났으며, Stroop 검사 중 단어읽기반응시간, 색단어읽기 반응시간, 색채 말하기반응시간, 색단어 단어 읽기반응 시간검사에서도 유의한 차이가 나타났다. 하지만 WCST 에서는 어떠한 차이도 나타나지 않았다. 기존의 지능에 따른 분류, 내재화 또는 외현화 문제 와 달리 본 분류에서는 발달지연군의 전반적인 기능이 떨어지는 것으로 나타났는데, 이는 신경인지기능의 수 행에 필요한 운동발달, 언어발달이 늦었던 것과 관련이 있을 것으로 생각한다. 향후 ADHD 환자를 평가, 치료 하는 데 있어서 정신병리를 함께 평가하는 것은 ADHD 아동의 치료 방법 및 예후 결정하는데 필요할 것으로 생 각한다.
Abstract
Objectives：This study was performed to evaluate neurocognitive function according to psychopathology in children with ADHD. Methods：All the participants were divided into 5 groups, development delay group(DDG), internalizing symptoms group(INT), externalizing symptoms group(EXT), combined symptoms group(COM), and ADHD only group(PAG) by the T scores of KPI-C. CNT scores of the five groups were compared. Results：Intelligence quotient and omission errors in auditory continuous performance test(CPT) were significantly lower in DDG than other groups. Time and errors in trail making test A and B in DDG were significantly higher than other groups. Reading black word, reading color word, color naming, word reading of color word in DDG were also significantly different than other groups. There’s no significant difference in six category of Wisconsin Card Sorting Test. Conclusion：This study demonstrates that overall neurocognitive function in DDG is lower than other groups of children with ADHD. It is postulate that DDG group of children delayed in verbal and motor development and it is related to poorer performance of neurocognitive function. Considering psychopathology in diagnosing children with ADHD is important to select treatment modalities, expect treatment response, and to anticipate long term prognosis
