알코올 사용 장애 환자에서 알코올 사용의 심각도에 따른 불안 및 우울 증상과의 연관성

초록

목적 : 알코올 사용 장애를 가진 남녀 환자들을 대상으로 공존질환과 동반되는 증상을 알아보고, 알코올 사용의 심각도에 따른 불안 및 우울 증상과의 관련성을 알아보고자 하였다. 대상 및 방법 : 총 85명의 알코올 사용 장애 환자(3명의 알코올 남용, 82명의 알코올 의존 환자)를 대상으로 하였고, 알코올 사용의 심각도를 평가하는 도구로는 한국어판 알코올 사용장애 진단 검사(The Korean version of alcohol use disorders identification test, AUDIT-K)와 한국형 알코올 중독 선별 검사(National Alcohol Screening test, NAST)를 사용하였으며, 불안 및 우울 증상의 평가를 위해서는 Beck 불안 척도(Beck Anxiety Inventory, BAI) 및 Beck 우울 척도 (Beck Depression Inventory, BDI) 를 사용하였다. 알코올 사용과 불안, 우울 증상과의 연관성을 알아보기 위하여 Pearson’s Correlation Test를 시행하였으며, 심각한 수준의 알코올 사용과 불안, 우울 증상과의 연관성을 추가적으로 알아보기 위해 AUDIT-K≥26인 환자들만을 대상으로 Pearson’s Correlation Test를 재 시행하였다. 결과 : AUDIT-K, NAST, BAI 와 BDI의 평균점수는 각각 22.22(±8.28), 6.24(±3.25), 18.64(±13.08), and 22.05(±10.13) 였다. 알코올 사용의 심각도를 측정하는 도구인 AUDIT-K와 NAST 사이에는 유의한 양의 상관관계를 보여주었다(r=.534, p<.01). AUDIT-K와 BAI 그리고 BDI 사이에서도 유의한 양의 상관관계를 보였으며(r=.231, p<.05; r=.334, p<.01), NAST와 BAI 그리고 BDI 사이에서도 유의한 양의 상관관계를 보였다(r=.251, p<.05; r=.419, p<.01). 알코올 사용 장애 환자들 중 AUDIT-K 점수가 26점 이상인 환자들을 대상으로 실시한 상관관계 분석에서는AUDIT-K와 BAI 사이에는 유의한 양의 상관관계를 보였지만 (r=.425, p<.05), AUDIT-K와 NAST 그리고 BDI 사이에서는 유의한 상관관계가 나타나지 않았다. 결론 : 이번 연구의 결과로 알코올 사용 장애 환자들에서 알코올 사용의 심각도와 불안 및 우울 증상과의 연관성을 확인할 수 있었다. 특히, 불안 증상의 경우 심각한 알코올 사용장애 환자에서 더 큰 연관성을 보였다. 우울 증상의 경우는 이와는 달리 일관성 있는 결과가 나오지 않았는데 이에는 사회심리학적인 요소가 작용했을 것으로 보이며 추후 연구가 필요하겠다. 알코올 사용 장애를 가진 환자를 치료할 때, 불안이나 우울 장애를 동반하고 있는지에 대해 면밀한 조사가 필요할 것으로 보이며, 이를 통해 알코올 사용 장애 환자들이 가지는 불안이나 우울 증상들을 조기에 치료할 수 있다면 알코올 남용이나 의존을 치료하는 데 있어서도 많은 도움을 줄 것이다.

Abstract

Objectives：The purpose of this study was to examine the relationships of anxiety&depressive symptoms according to the severity of alcohol use in patients with alcohol use disorder. Methods：A total of 85 patients with alcohol use disorders were enrolled in this study. The severity of alcohol use was measured by the Korean version of alcohol use disorders identification test(AUDIT-K)&National Alcohol Screening test(NAST). Anxiety and depressive symptoms were measured by using the Beck Anxiety Inventory(BAI) and the Beck Depression Inventory(BDI), respectively. Pearson’s correlation test was applied to correlate among the severity of alcohol use, anxiety and depressive symptoms. Results：The mean scores of AUDIT-K, NAST, BAI and BDI were 22.22(±8.28), 6.24(±3.25), 18.64(±13.08), and 22.05(±10.13), respectively. Pearson’s correlation test showed that between the score of AUDIT-K and NAST, BAI or BDI, there were positively significant correlations(r=.534, p＜.01 ; r=.231, p＜.05 ; r=.334, p＜.01), respectively. And in cases of≥26 score of AUDIT-K, there were much more positive correlations with BAI(r=.425, p＜.05). Conclusion：The results of this study demonstrated that patients with alcohol use disorders have high co-morbid anxiety and depressive disorders. Overall, the greater severity of alcohol use may be associated with more anxiety and depressive symptoms. However, it is suggested that in the patients with severe alcohol use, correlation appeared to be positive only for anxiety symptoms.
