목적：Clozapine의드문 부작용으로 췌장염이나 고혈당증 등이 보고되고 있지만 아직까지 정확한 기전은 밝혀지지 않고 있다.본연구는 이러한 부작용과 관련하여 clozapine이췌장 조직에 직접적인 세포 독성을 일으킬 것이라는 가설 하에,clozapine이HIT-T15 췌장 β-세포주에서 세포 사멸 과정인 apoptosis를유도하는지를 알아보고자 하였다. 방법：먼저MTT assay로HIT-T15 췌장β-세포주에서 clozapine의세포 독성을 알아보고 형태학적 변화를관찰하였으며 이러한 세포 독성의형태로apoptosis가일어나는지를 DAPI staining, TUNEL assay, caspase3 assay 등의 방법을 사용하여 알아 보았다. 결과：Clozapine이HIT-T15 췌장 β-세포주에서 농도와 시간 의존 양상으로 세포독성을 일으키고,형태학적관찰, DAPI staining, TUNEL assay 결과에서 apoptosis의특징적인 소견을나타내었다.그리고 clozapine처치 시 caspase-3의활성이 유의하게 증가되어,caspase-3에 의해 매개되어 apoptosis가 유도된다는 것을알 수 있었다. 결론：본 연구결과로볼때, HIT-T15 췌장β-세포주에서 clozapine에의해 유도되는apoptosis가 clozapine에 의해 발생되는 췌장염이나 고혈당증 같은 부작용의 병태생리학적 기전에 관여할 가능성이 있을것으로 생각한다.
Abstract
Obejectives： We hypothesized that clozapine has a direct cytotoxic effect on pancreatic tissue in relation to certain side effects such as clozapine induced hyperglycemia and pancreatitis. The present study was undertaken to investigate our hypothesis by detecting clozapine-induced cell death process, apoptosis in HIT-T15 pancreatic β-cell line. Methods： To investigate whether clozapine induces apoptosis in the cells of the HIT-T15 hamster insulinoma tumor cell line, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide(MTT) assay, terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling( TUNEL) assay, 4,6-diamidino-2-phenylindole (DAPI) staining, and casapse-3 enzyme assay were performed. Results： Through morphological and biochemical analyses, it was demonstrated that HIT-T15 cells treated with clozapine exhibit several features of apoptosis. Our results showed that, clozapine appears to activate specific intracellular death-related pathways, including possibly the caspase-3 activation pathway, and thus induce apoptosis in hamster insulinoma cells. Conclusion： From the present study, it can be suggested that clozapine-induced apoptosis of pancreatic β insulinoma cells is one possible basic mechanism of clozapine induced pancreatitis and hyperglycemia.
