호스피스 완화의료 미술치료의 현황 및 인식 연구 -다학제 팀원중심으로

본 연구는 국내 호스피스 완화의료 서비스 개선을 위한 요법치료로 실행되고 있는 미술치료의 현황 및 인식을 조사하는 데 목적이 있다. 이를 위하여, 전국 입원형 호스피스 완화의료 기관 76곳의 의사 8명, 간호사 127명, 사회복지사 83명, 요법치료사 27명 총 245명을 대상으로 설문조사를 진행하였다. 연구결과, 67개(88.2%) 기관에서 미술치료를 실행하고 이 중 49개(73.1%) 기관에서 미술치료사를 고용하고 있는 것으로 조사되었다. 호스피스 완화의료의 미술치료는 환자와 보호자에게 긍정적 영향을 미치는 것으로 나타났다. 이 중 환자가 자신의 무의식적 감정을 표현할 수 있도록 돕는 것이(M=1.50, SD=0.612) 가장 긍정적으로 평가되었다. 환자의 연령 및 신체기능 수준(M=1.79, SD=0.738)을 미술치료 계획 시 우선 고려해야 하고, 이를 바탕으로 프로그램 개발의(M=1.62, SD=0.619) 필요성이 높게 조사되었다. 미술치료사의 자질로서 호스피스 완화의료의 전문적인 지식(M=1.65, SD=0.916)이 가장 요구되는 것으로 파악되었다. 응답자의 호스피스 완화의료 표준교육 이수여부와 미술치료의 참관경험간 유의미한 연관성(𝑥²=9.383, p=0.004)이 있는 것으로 나타났으며. 참여, 참관 경험의 여부는 미술치료의 인식에 유의미하게 영향을 미치는 것으로 평가 되었다. 본 연구는 국내 호스피스 완화의료 미술치료의 현장에서 필요한 실질적인 서비스 개선을 위한 기초 자료를 제시한 것에 의의를 가진다.
Abstract
This study investigated the current status and recognition of art therapy in Korea's hospice palliative care institutions that may improve hospice palliative care services. The survey’s groups consisted of total 245 people from 76 inpatient hospice palliative care institutions: 8 doctors, 127 nurses, 83 social workers, and 27 therapists. As a result of this study, 67(88.2%) institutions are conducting art therapy and 49(73.1%) are engaging art therapists. Further, 159(64.9%) and 154(62.9%) responded positively to the impact on patients and care-givers through the use of art therapy. The most positive evaluation was helping patients express unconscious emotions(average=1.50, SD=0.612). The most important factors considered in the art therapy plan were the age and physical function level of the patient(average=1.79, SD=0.738). The Study found that there was a significant connection(𝑥=9.383, p=0.004) between the completion of hospice palliative care standard education and participation in art therapy or observation experience. This study is important for presenting basic data for practical service improvement required in the field. beyond that the study prepared basic data for follow-ups as a quantitative study on the current status in addition to recognition of the practice of hospice palliative care art therapy.
