미술치료사의 호스피스ㆍ완화의료 미술치료 경험에 대한 해석학적 현상학 연구

본 연구는 호스피스⋅완화의료 현장에서 미술치료를 수행한 미술치료사의 체험된 현상의 의미와 본질을 이해하는 데 목적을 둔다. 연구 참여자는 5명의 미술치료사이다. 면담은 참여자별로 1∼2회 진행되었고, 수집된 자료는 van Manen의 해석학적 반성과정을 통해 5개의 본질적 주제와 17개의 하위주제로 도출되었다. 미술치료 경험의 본질적 주제는 ‘호스피스 환자와 가족을 대하는 것이 어려운 관문이 됨’, ‘미술치료에 대한 몰이해와 예측할 수 없는 환자들의 상황’, ‘임종을 앞둔 환자와의 작업으로 인한 소진’, ‘미술치료의 의미와 보람을 찾음’, ‘죽음과 삶에 대해 성찰하고 미술치료사로서 성장함’이다. 본 연구의 결론은 다음과 같다. 첫째, 미술치료사는 호스피스⋅완화의료 현장을 접하는 것 자체만으로도 긴장하지만 환자와의 꾸준한 관계 형성을 위한 노력으로 이를 극복한다. 둘째, 임종 환자와의 작업으로 마음의 동요와 부담감을 느끼며 정서적으로 고갈된다. 셋째, 환자들이 통증을 잊고 가족과의 정서적 소통이 일어나는 것으로 미술치료의 치료적 힘을 체험한다. 넷째, 죽음과 삶에 대해 성찰하게 되고 삶의 소중함과 내담자를 바라보는 가치관의 변화를 경험하며 성숙해 간다. 본 연구는 호스피스⋅완화의료 미술치료에 대한 구체적 이해를 돕는 학문적 기초자료로 제공될 수 있으며, 미술치료사의 호스피스⋅완화의료 미술치료 경험의 의미와 미술치료 수행에 있어서의 제도적 개선점들을 살펴볼 수 있었다.
Abstract
The purpose of this study was to understand of the meanings and nature of the phenomena experienced by art therapists who performed art therapy in the field of hospice and palliative care. The participants were 5 art therapists. Interviews were conducted once or twice for each participant. The collected data were identified 5 essential themes and 17 subthemes through van Manen’s hermeneutic reflection process. The five essential themes of the experiences of participants were as follows: ‘dealing with hospice patients and their families became a difficult gateway in art therapy’, ‘a lack of understanding of art therapy and unpredictable patients’ situation’, ‘burnout due to art therapy for patients facing imminent death’, ‘finding the meanings and rewarding feelings for art therapy’, and ‘reflecting on death and life and growing as an art therapist’. The conclusions of this study are as follows. First, although art therapists feel nervous as they first experience the field of hospice and palliative care, they overcome their nervousness through their continuous efforts to build relationships with patients. Second, while conducting art therapy for patients facing imminent death, art therapists become agitated and feel pressure. Third, as art therapists observe patients forget pain and have emotional communication with their family, they experience the therapeutic power of art therapy. Fourth, as art therapists get to reflect on death and life, realize the value of life, and experience changes in their perspective and values for their clients, they become mature. This study can be provided as academic basic data to help concrete understanding art therapy in hospice and palliative care, and examined the meaning of art therapists’ experience and aspects institutional improvements in performing art therapy in hospice and palliative care.
