장기간 탈출된 전방인공수정체의 재삽입술

목적: 전방인공수정체의 지지부가 1년간 공막 밖으로 탈출된 환자에서, 전방에 재위치 시킨 1예를 보고하고자 한다. 증례요약: 64세 남자 환자로, 8년 전 안과의원에서 우안 백내장 수술을 받았으나 시력호전은 없었다고 하였다. 1년 전 다른 안과의원에서 큰 병원 진료 권유를 받았으나 지켜보다가 이물감과 시력저하 계속되어 본원을 내원하였다. 전방인공수정체의 지지부가 11-1시방향에서 공막 밖으로 튀어나와있고, 그 끝 부위 2곳은 결막 밖으로 노출되어 있었다. 나안시력 0.2, 안압 27 mmHg, 각막내피세포밀도가 1222 cells/mm2이었다. 공막 밖으로 탈출한 인공수정체 지지부를 박리한 후, 2-8시 방향으로 위치하도록 재삽입하였다. 수술후 6달째 우안 나안시력 0.3, 교정시력 0.8, 안압은 12 mmHg이었고 각막내피세포 밀도는 838 cells/mm2이었으며 염증소견 없이 눈은안정적 상태였다. 결론: 전방인공수정체의 지지부가 공막 밖으로 탈출된 경우, 전방인공수정체를 제거하지 않고, 앞방 내에 90도 회전시켜 삽입하는 수술법은, 간단하고 수술 후에 안정적인 상태를 유지하였다. 다만 각막내피세포밀도는 감소할 수 있음을 고려해야 한다.
Abstract
Purpose: To report a case of anterior chamber intraocular lens (ACL) reposition with the haptic protruded into the subconjunctiva in a patient with a previous ACL implantation. Case summary: A 64-year-old man visited our clinic because of visual disturbance and discomfort in his right eye. Approximately 8 years earlier, he had cataract surgery and there was no visual improvement but eye discomfort. The haptic of the ACL protruded into the subconjunctiva at 11-1 o’clock. The visual acuity of the right eye was 0.2 and the intraocular pressure of the right eye was 27 mmHg. The ACL was repositioned because of low cell density (1222 cells/mm2). After 6 months, the visual acuity of the right eye was 0.3, best corrected visual acuity was 0.8, intraocular pressure was 12mmHg and cell density was 838 cells/mm2. There were no inflammation signs or complications. Conclusions: Reinserting ACL when the haptic is protruding into the subconjunctiva could be a simple and safe method when a decrease in corneal endothelial cell density is present.
