무렌각막궤양 환자에서 시행한 자가 귀구슬연골막 이식술 1예

목적: 활동성 무렌각막궤양 환자에서 시행한 자가 귀구슬 연골막 이식술 1예를 보고하고자 한다. 증례요약: 49세 남자 환자가 무렌각막궤양 진단 하 본원으로 의뢰되었다. 내원시 궤양이 각막 변연부 360도 전체를 침범하고 있는소견을 보였으며 시력은 0.04였다. 내과적 치료에 반응하지 않고 궤양이 급속히 진행하는 소견을 보여 수술적 치료를 결정하였으며,침범된 각막과 결막을 절제하고 결손부위를 자가 귀구슬 연골막과 동종 각막 윤부 이식술을 시행하여 복원하였다. 수술 후 각막 궤양의 진행은 멈췄으며 주관적 증상 또한 뚜렷이 호전되었다. 수술 1주 후 이식편 위로 빠른 혈관형성이 이루어졌다. 그러나 중심부 각막의 혼탁으로 수술 5개월 뒤, 전층 각막 이식술을 시행하였으며, 각막이식 시행 후 6개월까지 저용량의 점안 스테로이드 투여만으로거부반응 없이 각막의 투명성이 유지되었고 윤부 기능 부전 소견은 없었다. 환자의 최종적 최대교정시력은 0.125였다. 결론: 무렌각막궤양의 수술적 치료에서 귀구슬 연골막 이식은 결손부위의 수복, 동반 이식편의 생착, 각막의 기능 유지에 효과적이었다. 따라서 각막이식의 예후가 불량한 활동성 무렌각막궤양과 윤부 기능 부전이 발생 가능한 질환에서 효과적인 치료법으로서 고려될수 있을 것이다.
Abstract
Purpose: To report the effectiveness of an autologous tragal perichondrium graft for an active Mooren’s ulcer. Case summary: A 49-year-old male was referred to our clinic with Mooren’s ulcer in his left eye. On the first visit, the ulcerinvolved the entire 360 degrees of the peripheral cornea and the visual acuity was 0.04. As the ulcer did not respond tomedical treatment and progressed rapidly, the authors decided on surgical treatment. First, the involved cornea and nearbyconjunctiva were removed and the defect was successfully reconstructed with autologous tragal perichondrium and allogeniclimbal graft. The progression of corneal ulceration ceased after surgery and the patient’s symptoms remarkablyimproved. Additionally, the graft showed rapid vascularization and engraftment within a week. However, as the opacityevolved and progressed to the central cornea, penetrating keratoplasty (PKP) was performed 5 months after the perichondrialgraft. There was no rejection and corneal translucency was kept clear with low-dose steroid eye drops until 6 monthsafter PKP without any sign of limbal deficiency. The patient’s final best corrected visual acuity was 0.125. Conclusions: The present study shows that autologous tragal perichondrium graft is an effective method of treatment foractive Mooren’s ulcer by defect reconstruction and results in survival of the co-transplanted graft as well as maintainingcorneal functions.
