사르코이도시스로 오인된 상안검에 발생한 뮬러근육속혈관종 1예

목적: 재발을 반복하는 눈꺼풀부종을 주소로 내원한 환자에서 누선염으로 오인된 뮬러근육속혈관종을 경험하였기에 이를 보고하고자한다. 증례요약: 특이병력 없는 21세 남자 환자가 2년 전부터 시작된 우안 윗눈꺼풀 부종과 발적, 우안 충혈과 눈곱을 주소로 본원 내원하여눈물샘을 침범하는 안와거짓종양 의증 진단하에 혈액검사 시행하였고, 안지오텐신 전환 효소가 53.0 U/L로 증가된 소견 보여 사르코이도시스 의심하여 절개생검을 시행하였다. 약 0.9×0.8 cm 크기의 경계가 지어지는 종괴가 뮬러근 사이에서 절제되었다. 조직검사결과 근섬유다발 사이에 적혈구가 가득 찬 다양한 크기의 혈관들로 구성되어 있어 뮬러근육속혈관종에 부합하는 소견을 보였다. 함께절제한 안와지방과 눈물샘은 정상조직소견을 보였다. 결론: 두경부의 근육속 혈관종은 드문 양성종양으로 종괴가 아닌 미만성 부종의 형태로도 나타날 수 있으며, 특히 안지오텐신 전환효소가 상승되어 사르코이도시스나, 뮬러근에 위치하는 경우는 누선염으로 오인될 수 있음을 염두에 두어야 하겠다.
Abstract
Purpose: We report a case of Muller muscle intramuscular hemangioma in the upper eyelid mimicking dacryoadenitis. Case summary: A 21-year-old male patient presented with a 2-year history of upper eyelid swelling in his right eye and no symptomrelief with oral corticosteroid. Increased angiotensin-converting enzyme (ACE) was noted in the laboratory findings. However, lid inflammation was not improved and the patient underwent incisional biopsy. Conclusions: Microscopically, the lesion was composed of proliferating vessels of various sizes between the individual smoothmuscle fibers. Intramuscular hemangioma rarely presenting as a benign tumor in the head and neck region can present as diffuseswelling mimicking sarcoidosis or dacryoadenitis.
