안와에 국한된 사르코이드증 1예

목적: 전신 침범 없이 안와에 국한된 사르코이드증 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 33세 여자 환자가 1개월 전 발생한 좌안의 안검 부종 및 발적을 주소로 내원하였다. 이학적 검사상 좌안의 내안각 및 상하안검 아래에 미만성 종괴 및 발적이 관찰되었으며, 안와 전산화단층촬영상 경계가 불명확한 조영증강이 앞쪽 안와의 내측 및 하부에관찰되었다. 악성가능성을 배제하기 위해 종괴에 대한 절제 및 생검을 시행하였고, 병리조직검사에서 비건락성 육아종성 염증이 관찰되었다. 이물질은 발견되지 않았고, AFB, methenamine silver 및 PAS 염색은 음성이었다. 흉부 방사선 검사상 특이소견 관찰되지않았고, 결핵균에 대한 PCR 및 인터페론감마분비 검사에서 음성 결과를 보였다. ANA 및 ANCA 검사에서 음성결과를 보였고, 혈청ACE 수치는 10.3 μ/L로 정상 범위 내였다. 이상의 소견으로 안와 사르코이드증 진단하에 경구 스테로이드를 처방하였고, 치료 6주째안검부종 및 발적 사라졌고, 치료 시작 후 7개월까지 재발 없이 유지되는 양상 보였다.
Abstract
Purpose: The authors report a case of orbital sarcoidosis without evidence of systemic involvement. Case summary: A 33-year-old female had a 1 month history of erythematous eyelid swelling. On physical examination, a firmand non-tender mass was observed diffusely along the upper, lower and medial canthal areas. A computed tomography (CT)scan showed a diffuse mass in the anterior orbit. We performed an incisional biopsy and histopathological examination revealednon-caseating granulomas and no evidence of a foreign body. Acid-fast-bacilli (AFB), methenamine silver and periodic-acidschiff(PAS) stain showed no evidence of infection and chest radiograph was normal. Polymerase chain reaction (PCR) and interferongamma secretion test showed no evidence of tuberculosis. Antinuclear antibody (ANA) and antineutrophil cytoplasmicantibody (ANCA) were negative and angiotensin converting enzyme (ACE) was within the normal range. Further systemic evaluationswere compatible with a diagnosis of orbital sarcoidosis and oral prednisone was prescribed. Six weeks later, the erythematouseyelid swelling had disappeared and there was no evidence of recurrence to date.
