목적: 눈물배출계에 발생한 골수외 형질세포종 1예를 경험하고 치료하였기에 보고하고자 한다. 증례요약: 56세 여자가 1년 전부터 시작된 우안의 눈물흘림과 안쪽눈구석에 압통이 없고 고정된 양상을 보이는 종괴가 촉지되어 내원하였다. 안와전산화단층촬영에서 종괴가 우측 눈물주머니와 코눈물관을 따라 길게 위치하고 있었으며 비강 내에도 침범되어 있었다. 이비인후과에서 종괴 생검을 시행하였으며 병리조직검사에서 골수외 형질세포종으로 진단되어 1개월 동안 25회에 걸쳐 총 4,500cGy의 방사선 치료를 받았다. 28개월의 경과관찰에서 부비동전산화단층촬영을 통해 종괴과 소실되고 재발되지 않은 것을 확인하였으며 코내시경에서도 비강 내의 특이한 병변은 관찰되지 않았다. 결론: 눈물배출계에 발생한 골수외 형질세포종을 경험하고 방사선치료로 좋은 결과를 보였기에 이를 보고하는 바이다.
Abstract
Purpose: To report a case of extramedullary plasmacytoma in the lacrimal excretory system. Case summary: A 56-year-old female patient suffering from chronic epiphora for 1 year and having a non-tender, fixed palpablemass in the medial canthus of the eyelid visited the outpatient clinic. Orbital CT showed the non-enhancing mass was locatedalong the right lacrimal sac and nasolacrimal duct and protruded into the nasal cavity. Biopsy was performed at theOtolaryngology Department. Based on histopathological examination, the mass was diagnosed as extramedullary plasmacytomaand the patient was treated with a total dose of 4500 cGy radiation (25 times in 1 month). After the 28-month follow-up period,the mass was not observed on Sinus CT and there was no recurrence. Additionally, no specific lesion was found during nasalendoscopy. Conclusions: We report a case of extramedullary plasmacytoma in the lacrimal excretory system successfully treated by radiationtherapy.
