천식 치료와 호흡곤란 인지능력의 변화

호흡곤란(dyspnea)은 폐 및 기관지 질환의 가장 중요하고도흔한 증상 중 하나이다. 호흡곤란은 여러 가지 생리적, 정신적, 환경적 요소들이복합적으로 관여하여 만든 ‘숨이 원활하게 쉬어지지 않는다’는 주관적인 증상의 인식이다. 호흡곤란의 생리적 기전이나인지하는 감각 경로는 여전히 불분명하다. 호흡곤란이란 용어 자체도 다소 병적 의미를 포함하고 있어, 정상인이 운동과 같은 자극에 의해 숨이 가쁘다고 하는 정상 생리적 현상의 표현과는 차별화 되어 사용되어왔다. 또한 호흡곤란이 여러 가지 다른 감각들이 복합적으로 인지되어진 것인지, 하나의 특이한 감각이 인지된 것인지도 잘 모른다.1) 천식과 같은질환을 가진 환자에게서 호흡곤란을 인지하거나 그 심한 정도를 인식하는 것은 환자 스스로 치료를 받고자 하는 동기로작용하므로 매우 중요하다. 기관지 수축이 발생하였을 때 그에 적합한 호흡곤란을 환자 스스로 인지 하지 못한다면 치료가 늦어지거나 치료 강도도 약해질 수 있기 때문이다.2)
