천식은 우리나라를 비롯한 세계 대부분의 지역에서 유병률이 약 5∼18%에 달하는 만성 호흡기 질환으로서, 사회 경제적 비용의 증가와 밀접한 연관을 가지고 있다. 국제적 천식치료 지침(Global INitiative for Asthma)에 따르면 천식은 여러 세포와 다양한 매체들이 관여하는 기도의 만성 염증성 알레르기질환으로, 기도과민증과 이와 연관되어 발생하는 반복적인 천명, 호흡곤란, 가슴 답답함, 기침 등의 증상을 일으키는 질환으로 정의한다. 과거에 비하여 기도염증의 중요성이 재차 강조되고 있으나 실제 임상에서 기도염증을 객관적으로 측정하는 방법은 아직까지 확립되어 있지 않으며, 기도염증 또한 환자의 상태에 따라서 가변적일 수 있다. 최근에는 병인기전을 바탕으로 천식을 단일 질환이 아닌 다양한 특성의 하위그룹을 포함하는 복합적인 질환군으로 이해하고 있다.1) 이와 같은 개념에 따라 여러 코호트 연구에서는 다양한 임상지표를 변수로 한 군집분석(cluster analysis)을 이용하여 천식을 몇 가지 표현형으로 분류하고 있다.2) 여기서 천식의 표현형(phenotype)이라고 함은 자연경과, 임상적, 생리학적인 특성 등이 일관적인 양상이고 이를 뒷받침하는 공통된 병태생리와 이와 연관된 바이오마커 및 유전형을 보여주고 있으며, 기전과 연관된 치료에 예측 가능한 반응을 보이는 것으로 정의하고 있다.3) 저자마다 차이가 있으나 대표적으로 다음과 같은 다섯 가지 표현형, 즉 조기발생 알레르기성(early onset allergic asthma), 후기발생 호산구성(late-onset eosinophilic), 운동 유발성(exercise-induced), 비만 연관성(obesity-related), 호중구성(neutrophilic)으로 분류하고 있다.
