Extracellular vesicles (EVs) released from different types of kidneycells under physiologic conditions contribute to homeostasismaintenance, immune-modulation, and cell-to-cell communications. EVs can also negatively affect the progression of renaldiseases through their pro-inflammatory, pro-fibrotic, and tumorigenicpotential. Inhibiting EVs by blocking their production, release, and uptake has been suggested as a potential therapeuticmechanism based on the significant implication of exosomesin various renal diseases. On the other hand, stem cell-derivedEVs can ameliorate tissue injury and mediate tissue repair byameliorating apoptosis, inflammation, and fibrosis while promotingangiogenesis and tubular cell proliferation. Recent advancementin biomedical engineering technique has made it feasible tomodulate the composition of exosomes with diverse biologicfunctions, making EV one of the most popular drug deliverytools. The objective of this review was to provide updates ofrecent clinical and experimental findings on the therapeuticpotential of EVs in renal diseases and discuss the clinical applicabilityof EVs in various renal diseases.
