임상에서 활동 중인 종양전문간호사의 직무분석-DACUM 기법 이용-

본 연구는 DACUM 기법을 이용하여 국내에서 현재 활동 중인 종양전문간호사의 직무를 분석한 조사연구로서, 종양전문간호사의 표준화된 직무기술 및 역할 재정립을 위한 기초 자료를 제공 하고자 계획되었다. 먼저 DACUM 워크숍과 타당도 검증을 통해 종양전문간호사의 직업 정의 및 직무내용을 도출하였고, 이에 대한 빈도, 중요도, 난이도를 측정하기 위해 전국400병상 이상의 병원에서 활동하고 있는 종양전문간호사 53명을 대상으로 설문조사를 하였다. 본 연구의 결론은 다음과 같다. 첫째, 종양전문간호사는“임상현장에서 암환자와 가족을 대상으로 치료결과와 삶의 질 향상을 위하여 전문적 간호 실무를제공하여 암환자 간호 발전을 위해 리더쉽을 발휘하는 자”로정의되었고, 직무내용은 12개의 임무와 66개의 작업으로 구성되었다. 둘째, 12개의 임무 중 빈도가 가장 높은 것은“환자 및 가족교육”, 중요도가 가장 높은 것은“임상적 판단과 의사결정”이었으며, 난이도가 가장 높은 것은“상황별 암환자 사례관리”였다. 셋째, 66개의 작업 중 빈도가 가장 높은 것은“환자 및 가족교육하기”, 가장 낮은 것은“암 예방 및 조기발견 프로그램 운영하기”였고, 중요도가 가장 높은 것은“환자 및 가족 교육하기”와“환자상태 사정하기”, 가장 낮은 것은“홈페이지 관리하기”였고, 난이도가 가장 높은 것은“독자연구 수행하기”, 가장낮은 것은“항암제 투여하기”였다. 이상의 결론을 토대로 다음과 같은 제언을 하고자 한다. 첫째, 본 연구는 최근 임명된 종양전무간호사가 제외되었고,일부 응답을 하지 않았기 때문에 보다 많은 수의 종양전문간호사를 대상으로 한 반복 연구가 필요하다. 둘째, 본 연구는 직무내용을 임무와 작업으로 기술하였는데향후 반복 연구에서는 작업요소(task element)까지 기술해본다면 종양전문간호사의 직무를 좀 더 구체적으로 확인할 수 있을 것이다. 셋째, 본 연구의 직무내용은 실무자의 현장경험을 토대로 도출된 것이므로, 간호계 내에서 종양전문간호사의 직무기술을표준화하고 재정립하는데 활용할 것을 제언한다. 넷째, 각 병원마다 종양전문간호사의 활동영역이나 역할이다양하기 때문에 본 연구에서 나온 직무내용을 기초로 하여 각자의 직무기술서를 작성하거나, 보완해볼 것을 제언한다. 다섯째, 종양전문간호대학원에서 교과과정을 개발하고 보완하는데 있어 기초 자료로 활용할 것을 제언한다. 여섯째, 종양전문간호사의 직무수행능력을 유지하고 향상시키기 위해서는 개인의 노력뿐만 아니라 각 의료기관이나, 학교, 간호협회 차원에서 교육프로그램 개발하고 제공하는 것이필요하다.
Abstract
Purpose : The purposes of this study were to identify job definition, duties and tasks of Korean oncology APNs (Advanced Practice Nurses) and assess frequency, importance, and difficulty for each of duty and task using the DACUM method. Methods : The DACUM committee was organized with nine oncology APNs. The committee documented the job definition,duties and tasks based on their daily job descriptions. Validity of the elicited duties and tasks was tested by a study team and 6 oncology APNs. Data were collected using a questionnaire survey from 53 oncology APNs. Results : The job description of oncology APNs was identified 12 duties and 66 tasks. Among the twelve duties, ‘education for patients/families’ was the most frequently performed and was the most important duty. ‘Case management’ was the most difficult duty. Among 66 tasks, ‘providing education for patients/families’ was most frequently performed. ‘Providing education for patients/families’ and ‘assessing patient condition comprehensively’ were the most important tasks. ‘Implementation research’ as the principal investigator was perceived as the most difficult task. Conclusion : In this study, Korean oncology APNs’ job performances were identified and analyzed. It is recommended that job description for oncology APNs needs to be standardized and reshaped in advanced way in future.
