Abstract
Oral tissue integrity refers to the balanced physiological state of the oral mucosa, teeth and periodontal tissues, salivary glands, and the oral microbiome, and is closely linked to systemic inflammatory and metabolic diseases. Although synthetic antiseptics such as chlorhexidine and cetylpyridinium chloride are widely used as the main active ingredients in conventional mouthwashes and provide short-term antimicrobial effects, their limitations—including mucosal irritation, taste disturbance, xerostomia, tooth staining, disruption of the oral microbiome, and potential induction of antimicrobial resistance—have been increasingly recognized. Consequently, there is growing interest in biocompatible alternatives that can modulate inflammation and tissue damage while preserving microbial homeostasis. This review focuses on plant-derived nanovesicles (PDNVs), an emerging class of natural extracellular-vesicle-like nanoparticles, and examines their biological properties, mechanisms of action, and recent research trends, with the aim of exploring their potential as novel active ingredients in mouthwash formulations. Literature analysis reveals that PDNVs derived from edible and medicinal plants such as turmeric, aloe vera, ginger, neem, garlic, and peppermint possess a lipid bilayer structure that encapsulates miRNAs, proteins, lipids, and antioxidant metabolites. These vesicles exhibit multiple biological activities, including suppression of inflammatory signaling, reduction of oxidative stress, modulation of microbial communities, and promotion of wound healing and tissue regeneration. Notably, garlic- and ginger-derived PDNVs have been shown to regulate periodontitis-associated pathogens and inflammatory responses, supporting their potential as functional components for protecting the oral mucosa and periodontal tissues. However, challenges remain, including the lack of standardized isolation and purification methods, limited safety and toxicity evaluations, insufficient clinical evidence for long-term oral exposure, and the absence of established regulatory frameworks. With further advancements in preclinical and clinical research and formulation engineering, PDNVs may emerge as a nature-derived nanotherapeutic platform that complements synthetic antiseptics in maintaining oral tissue integrity and preventing or managing periodontal diseases.
