한국 중년의 사회경제적 요인과 자살행동의 관련성 -성별 차이를 중심으로-

초록

목적: 본 연구는 지역사회 거주 중년의 자살생각 및 자살시도와 관련된 사회-경제적 요인을 탐색하고자 실시되었다. 방법: 연구대상은 2017년도 지역사회건강조사에 참여한 40~64세 106,331명이었다. 자살생각과 자살시도 문항을 활용하여 비자살생각군, 자살생각군, 자살시도군으로 분류, 복합표본 로지스티회귀분석을 통해 자살생각 및 자살시도 관련 요인을 분석하였다. 결과: 중년 남성의 자살시도 위험요인은 40~50대, 가구소득이 낮을수록, 과거 또는 현재 국민기초생활수급가구였으며, 낮은 가족 및 이웃 연락 빈도는 자살시도를 낮추는 요인으로 나타났다. 중년 여성의 경우 1인가구, 낮은 가족 연락 빈도, 그리고 정기적으로 참여하는 모임이 0~1개인 경우 자살시도 위험을 높였다. 결론: 중년여성은 낮은 사회적 연결망(1인 가구, 낮은 가족 연락 빈도, 낮은 사회참여)이 자살시도의 원인이었다. 반면 중년남성은 인구학적, 경제적, 사회적 연결망 등 다양한 요인이 자살에 영향을 미쳤다. 특히 사회적 네트워크는 자살생각과 자살시도 요인분석에서 남녀 간 상반되는 효과를 보였으므로, 이런 차이를 설명할 수 있는 후속연구를 제안한다.

Abstract

Objective: This study was conducted to explore socioeconomic factors associated with suicidal ideation and suicide attempts among community-dwelling middle-aged adults. Methods: The study population included 106,331 individuals aged 40-64 years who participated in the 2017 Community Health Survey. Using the suicidal ideation and suicide attempt questionnaire, the participants were categorized into non-suicidal, suicidal, and suicidal attempt groups, and factors associated with suicidal ideation and suicide attempt were analyzed using complex sample logistic regression. Results: Among middle-aged men, the risk factors for suicide attempts were 40s and 50s, lower household income, the past or current recipients of the National Basic Livelihood Security System, and low frequency of contact with family/friends. Among middle-aged women, living alone, low frequency of contact with family, and and lack of regular social activities were associated with an increased risk of suicide attempts. Conclusions: Among middle-aged women, low social network (living alone, low family contact frequency, and low social participation) were associated with suicide attempts. In contrast, a variety of factors, including demographic, economic, and social networks, influenced suicide attempt among middle-aged men. In particular, social networks had opposite effects in the factor analysis of suicidal ideation and suicide attempts, and I suggest further research to explain these differences.
