본 연구는 한국의료패널자료(Korea Health Panel Survey)를 활용하여 40세 이상 인구의 의료서비스 이용 및 의료서비스 구매에 영향을 미치는 요인들을 살펴본다. 본 연구는 앤더슨의 행동모형을 기반으로 한 건강모형을 구성하고, 소비자의 의료서비스 이용에 영향을 미치는 요인들을 선행요인, 활성화요인, 필요요인으로 구분하여 분석하였다. 이 방법은 의료서비스의 필요요인을 개인 및 사회경제적 요인을 구분함으로써, 의료서비스에 대한 접근성을 높이는 요인에 대한 중요한 시사점을 제공할 수 있는 분석틀을 제공한다. 본 연구에서는 합동회귀분석(pooled OLS)을 통해 개인의 인구적 특성에 따른 의료서비스 이용 패턴의 차이를 분석함과 동시에, 고정효과 패널분석(fixed effect panel analysis) 방식을 채택하여 건강함수 분석에 잠재하는 내생성 문제를 해결하고자 하였다. 합동회귀분석 결과는 개인의 성별, 학력수준, 가족 수에 따라 의료서비스 이용의 패턴에 차이가 있음을 보여준다. 이와 함께 고정효과 패널분석 결과는 개인의 건강상태 및 건강행동을 포함하는 소비자의 주관적인 체험이 의료서비스의 이용 및 의료서비스에 대한 구매에 대한 중요한 예측변수임을 보여준다. 특히 연령과 만성질환갯수, 주관적으로 평가한 건강상태 등과 같은 변수들은 의료서비스의 이용의 필요를 예측하는 주요한 변수임을 보여준다.
Abstract
Using the Korea Health Panel Survey data, this study examines the factors that incurs the consumption of medical service uses among Korean adults aged above 40. Based on the Andersen’s behavioral model, this study categorizes the determinants of medical service uses among Korean adults into three groups; predisposing, enabling and need factors. Categorizing various factors into individual versus socio-economic variables helps one to find important insights on how to expand general access to medical services among the elderly. Using the pooled OLS framework, this study shows that the patterns of medical service uses differ by individual’s demography characteristics. Moreover, this study overcomes the shortcomings of previous research by adopting the fixed effect(FE) panel analysis model. The FE panel analysis reveals that the customers’ subjective experiences including individual’s health status and health behavior variables are the main predictors of medical service uses among elderly. In particular, this study reports that individual’s health status variables such as age, the number of chronic diseases, and self-reported health status are the most important predictors of one’s need for medical service uses.
