‘돌봄’은 오랜 간호 역사의 전통을 통해 지금까지 이어져 온 윤리적 개념들인 옹호와 책임 및 협동을 포괄하며, 간호윤리학자들에 의해 가장 핵심적인 윤리적 개념으로 사용되고 있다. 특히 간호학자 왓슨은 돌봄을 간호의 도덕적 이상으로서 중심 개념으로 설정하고, 의료윤리와는 구별되는 간호윤리의 고유한 개념으로 사용하고 있다. 그녀는 ‘인간 돌봄이론’을 제안함으로써 간호학의 이론적 토대를 마련한다. 왓슨의 돌봄이론은 간호학에 철학적인 기반을 제공했고, 나아가 돌봄을 통해 다양한 간호현상을 이해하는 데 적극적 기여를 한 것으로 평가 받고 있다. 본 논문의 주된 목적은 왓슨의 인간 돌봄이론에서의 ‘초개인적 돌봄’의 가치체계와 왓슨이 자신의 이론의 철학적 토대로 삼고 있는 레비나스의 얼굴의 윤리를 검토함으로써 그녀의 돌봄이론이 이원론적 사유와 형이상학적 초월이라는 난점을 안고 있음을 드러내는 데 있다. 동시에 필자는 왓슨이 돌봄이론의 가치체계인 ‘카리타스 요소’와 ‘카리타스 과정’을 추적함으로써 간호에서 도덕적 이상으로서 돌봄이 드러내는 한계점을 지적할 것이다. 간호에서 돌봄을 도덕적 이상이라는 관점에서 ‘윤리’라는 이름으로 부르려고 한다면, 어떻게 그것이 ‘보편적인 규범적 강제성’을 가질 수 있는지 되물어볼 필요가 있기 때문이다.
Abstract
The concept of ‘Caring’ which comprehends advocacy, responsibility, and cooperation, has played a key role throughout the long tradition of nursing, and recent nursing ethicists commonly use it as a core ethical concept. Especially, Watson sets out caring as a central concept in terms of a moral ideal of nursing, and she uses it as a distinctive concept of nursing ethics, disparate from medical ethics. By presenting “human caring theory,” Watson has offered the philosophical foundation of nursing. Furthermore, Watson’s theory is regarded as a positive contribution to understanding the diverse nursing phenomena in terms of caring. The main aim of this paper is to show that Watson's human caring theory is grounded on the mind-body dualism and an ungrounded metaphysics, which is to give rise to difficulties in the practice of nursing. To do this, I will examine the value system of transpersonal caring in Watson’s theory, which draws heavily on Levinas's ethic of face. And I will point out the difficulties of caring as the moral ideal in nursing by explicating Watson’s notions of ‘Carative factor’ and ‘Caritas process’ of value system. According to an experientialist account, for a theory to bear the name of ‘ethics’ properly, it should justify itself as bearing a ‘universal normative compulsion’. In this respect, Watson’s human caring theory offers far more than “the compulsory”, which will give rise to intractable difficulties in the practice of nursing. I suggest that for Watson’s human caring theory to make more workable sense as a form of practical ethics, it should avoid some metaphysical assumptions.
