폭력 위험성 평가 도구 타당도 검증:(HCR-20 version 3)를 중심으로

폭력 위험성 평가 도구(Historical, Clinical, Risk Management-20: 이하, HCR-20)는 구조화된 전문가 판단(Structured Professional Judgement : 이하, SPJ) 계열의 도구로 폭력 위험성을 평가하는 측정 도구이다. 최종 목적이 재범률 예측인 기존의 보험 계리적 위험성 평가 도구와는 달리 SPJ를 기초로 한 HCR-20, RSVP 등과 같은 도구들은 위험 요인 뿐 만 아니라 사례 개념화와 시나리오 구성, 치료 및 위험 관리 등을 통해 전문가들이 자유 재량적으로 현재 위험 요인이 향후 발생될 수 있는 폭력위험성과 어떤 영향과 관련이 있는지를 한 번 더 생각해 볼 수 있다. 본 연구는 2013년에 개정된 버전 3을 이용하여 전국 7개의 수용시설에 수감된 재소자 27명을 대상으로 PCL-R(Psychopathy Checklist-Revised, 조은경, 이수정, 2008) 간의 공인타당도를 확인하고 평가자 간 신뢰도와 마지막 단계인 전문가 최종 의견 항목에 대해 평균 백분율 일치도(average percentage agreement)를 살펴보았다. 분석 결과, HCR-20V3의 총점과 PCL-R의 총점 간의 유의미한 상관관계가 나타났고, 훈련과 슈퍼비전을 받은 3명의 평가자 간 신뢰도에서도 전체적으로 유의미한 일치도를 보였다. 마지막 7단계 전문가 최종 의견의 4가지 항목에서는 평균 백분율 일치도에서는 72% 이상의 높은 일치성을 나타내었다. 마지막으로 이러한 연구 결과의 바탕으로 앞으로 국내에서의 폭력 위험성 평가도구의 활용 방안과 추후 연구에 대해 논의하였다.
Abstract
This study explores the performance of 27 incarcerated violent offenders across 7 correctional institution on the revised HCR-20, which is version3 and PCL-R(Psychopathy Checklist-Revised) to examine the concurrent validity between these two risk assessment instruments. Moreover, I evaluated the inter-rater reliability of the HCR-20V3 by three raters for 27 cases for ratings on the H, C, R scales and Summary Risk Ratings(SRRs) using interclass correlation coefficients(ICCs). We also calculated the average percent agreement between sets of pair-wise ratings for SRRs. Finding indicated that HCR-20V3 and PCL-R were significant correlated. Inter-rater reliability was significant for the most part strongly good for the presence of risk factors and for SRRs. Furthermore, it was suggested that predictive validity of the HCR-20V3 tests would also be needed for the korean violent offenders in the future.
