고부갈등으로 인하여 우울증을 겪고 있는 부인의 부부치료 사례연구

초록

본 연구는 우울증으로 신경정신과에 3주 동안 입원한 부인을 치료한 사례연구로서, 연구자는 부부상담과 개인상담을 병행하여 총 7회의 상담 과정을 서술하였다. 부인은 시부모와 갈등관계를 가지고 있었고, 시댁과 관련하여 남편과 관계가 좋지 않아 우울증이 심각한 상태였다. 남편은 2남 1녀 중 장남으로서 결혼을 하였음에도 불구하고 부모로부터 분리가 되어 있지 못하였다. 남편은 보수적이고 부모에게 매우 순종적인 사람이며, 부인은 독립적이며 원가족으로부터 비교적 분화가 잘 되어 있었다. 이 부부는 결혼한 지 1년이 되었으며, 슬하에 한 살 된 자녀를 두고 있었다. 치료의 초점은 남편을 원가족으로부터 분리시키고, 부인과 의사소통을 원활하게 하는데 두었다. 한편, 부인에 대한 치료의 초점은 남편과의 기능적인 의사소통을 하는데 두었다. 치료의 효과를 측정하기 위해서 기존에 사용되고 있는 측정도구들을 활용하여 결혼만족도 및 의사소통, 성만족도, 그리고 배우자 가족과의 관계의 변화를 살펴보았다. 한편, 치료과정의 변화를 살펴보기 위해서 대화분석법을 시도하였으며, 대화의 서술과 탐구분석의 효과적인 전시를 위해서 효과에 근거한 매트릭스(Effect Matrix)(Miles & Huberman, 1994, p. 96)와 시간의 변화에 따른 전시법(Time-ordered Meta Matrix)(Miles & Huberman, 1994, p. 200)을 활용하였다. 총 7회의 상담을 통하여 남편은 원가족으로부터 자아를 분화시키려고 노력하였으며, 남편은 부인과의 의사소통방법에 많은 변화가 일어났다. 한편, 부인은 남편에 대한 의사소통의 변화로 인하여 남편에 대한 인식의 변화가 있었으며, 부부관계가 원만하게 변화되었다. 결과적으로 부인은 부부관계의 변화로 인하여 고부간의 갈등으로부터 오는 스트레스가 감소되었고, 우울증도 사라지게 되었다. * 주제어: 고부갈등 우울증, 부부치료, 자아분화, 의사소통

Abstract

This case study investigated the change process of couple therapy for wive with depression. The client, who was suffering depression, was hospitalized at psychiatric hospital for 3 weeks. However, she did not show any improvement of her depression. The researcher executed individual, and couple therapies for husband and wife to improve wife's depression. The total sessions of therapy were consisted of 7 sessions and the average hours of therapy ranged from one hour and to one and half hour. The therapist attempted to differentiate her husband from his family of origin, and to change husband's dysfunctional communication pattern toward his wife. Also, the therapist tried to change wife's dysfunctional communication pattern into functional and effective communication pattern. The researchers used 5 instruments to measure the effects of couple therapy. The researchers used quasi-experimental research design and executed pretest(before first session) and posttest(after seventh session). Also, to complement of the weakness of the quantitative method, the researchers applied Effect Matrix(Miles & Huberman, 1994, p. 96) and Time-ordered Meta Matrix(Miles & Huberman, 1994, p. 200) as a qualitative research method to analyze the change process of couple's relationships.. The findings of research showed the decrease of wife's stress with her husband.and the improvement of wife's depression.
