뇌병변장애인의 심리사회적 특성 연구:장애영역별 비교

출생시 혹은 생애 어느 시기에 발생하는 장애는 개인 삶의 적응상 중요한 과제와 과정들을 요구한다. 조기 장애발생은 자기옹호 기술들을 강화할 필요를 요구하는 반면, 후기 장애발생은 친구와 가족에 미치는 영향 등 심리적 트라우마가 더 크기 때문에 교육적 접근이 필요하다는 입장이 있지만 재활에서의 전제는 개인에 따라 적응과정이 모두 다르다는 입장이다. 각종 통계가 보여주듯 뇌성마비를 포함하는 뇌병변 장애는 여러 가지 위험 요인들로 인해 그 발생율이 점차 증가하고 있다. 차별을 법적으로 금지하는 인권의 시대를 맞이하여 장애유형 간의 서비스격차에서 나타나는 실천적 문제들을 해결해야 할 시점에 와 있지만 상대적으로 정책적 임상적 차원의 사회배려가 부족하였던 뇌병변장애인 삶의 질을 높이고 사회 참여를 촉진시킬 수 있는 기초 자료는 여전히 부족하다. 이들이 직면한 곤경과 실상에 대해 제대로 이해하는 것이 전문가, 일반대중, 정책수립자, 담당공무원 뿐만 아니라, 특히 임상전문가들에게 있어서 중요하다. 이 연구는 우리나라 남동부에 거주하는 재가 뇌병변장애인 만20세 이상 132명과 20세미만의 뇌병변장애를 지닌 아동과 청소년 가정의 부모 등 대리인 49명에 대한 조사를 통해 집단별 뇌병변장애인이 직면한 문제해결을 위한 효과적인 방법을 찾기 위한 노력으로 9가지 영역의 조사결과와 주요 변수들 사이의 관계를 분석하였다. 연구결과를 통해 서비스의 확장과 더불어 연속적이고 원스톱 방식이 전개되도록 지역에서 가까운 곳에 소규모 프로그램들을 다양화하는데 필요한 제안과 개별 장애인의 특성에 부합되는 생활밀착형 서비스가 가능하도록 만드는 추가적 서비스와 그 방향성들을 논의하였다.
Abstract
This study was aimed at investigating subjective life satisfaction, discriminatory experience of various personal and social areas, subjective welfare needs, the activities of daily living (ADL), the instrumental activities of daily living (IADL) and current situations of leisure and vocational life for persons with brain lesion. Using a survey questionnaire structured on the above important research variables, data from 181 research participants were analyzed. Within this legal disability, three groups of disability, cerebral palsy, stroke, and traumatic brain injury (TBI), were compared on several variables selected and factors derived by multiple factor analyses. In terms of the ADLs and IADLS, persons with stroke showed high levels functional abilities followed by persons with CP and TBI. In addition, the participants' support needs were more influence by IADL than ADL among three groups of brain lesion. These research results are indicative of deliberated rehabilitation service planning and the individual service delivery model. Further research efforts should be more focused on the qualitative aspects of rehabilitation services and counseling with this most discriminated special legal disability group socially.
